AMENDMENT TO SERVICES AGREEMENT
NORTH CAROLINA
THIS AMENDMENT, made and entered into this the 25th day of Septemmber, 2015, by and between the County
of Orange, a body politic and corporate of the State of North Carolina, ("County"), and Courtney Kennedy
("Provider");
WITNESSETH:
WHEREAS, the County and Provider entered into a Services Agreement dated, July 1, 2015, to provide services to
be rendered by Provider to County's Health Department("Original Agreement"); and
WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all terms
and conditions of the Original Agreement not inconsistent with the terms and conditions set forth below.
NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the parties
agree to amend the Original Agreement as follows:
Section 4 Duration of Services
a. Term. The term of this Agreement shall be from July 1, 2015 to December 31, 2015.
Section 5. Compensation
a. Compensation for Basic Services. Modify the sentence as follows: The maximum amount payable for
Basic Services shall not exceed Twenty-Nine Thousand Seven Hundred Dollars ($29,700).
d. Travel. Modify the sentence as follows: Travel reimbursement shall not exceed Seven Hundred Fifty
Dollars ($750).
e. Supplies. Add the following sentence: Provider may be reimbursed up to an additional One Hundred
Twenty-Five dollars ($125) for necessary project-related expenses during the period October 1 through December
31s` provided that Provider demonstrates that she has expanded the monies previously distributed to her for
supplies.
Except for the changes made to Sections 4.a., 5.a, 5.d., and 5.e. herein, the Original Agreement shall remain in full
force and effect to the extent it is not inconsistent with this Amendment. In the event that there is a conflict
between the Original Agreement and this Amendment, this Amendment shall control.
IN WITNESS WHEREOF, Orange County and the Consultant have signed this Amendment, effective this
the 25 day of September, 2015.
