PURPOSE: To consider making appointments to the Adult Care Home Community Advisory
Committee.
BACKGROUND: The following information is for Board consideration:
NOTE: These positions are all 2 year terms with a maximum of 3 terms (6 years total).
• Appointment to a third full term (Position #2) At-Large DD/MR for Daniel Hatley expiring
10/31/2017.
• Appointment to a second full term (Position #4) At-Large for Max Mason expiring
06/30/2017.

RECOMMENDATION(S): The Manager recommends that the Board consider making
appointments to the Adult Care Home Community Advisory Committee.

Dear Thom,
Mr. Daniel Hatley's current term to the Adult Care Home Community Advisory Committee will be expiring on
10/31/15. We are recommending that Mr. Hatley be reappointed for another full term. During this past year
Mr. Hatley attended 4 out of 6 business meetings. For the two meetings that he missed he was excused. The
previous year he attended 100% of the business meetings and served as the Chair. He also participated in all
of the quarterly facility visits assigned to his subcommittee and supported several projects.
Mr Hatley is committed to the goals of this committee. He is a keen observer of concerns related to resident
rights during facility visits and is devoted to the enhancement of quality care and life of the elderly and
disabled.

Max Mason's current term to the Adult Care Home Community Advisory Committee will be expiring on 6/30/15. We
are recommending that Mr. Mason be reappointed for a second full term. Mr. Mason has provided strong leadership to
the committee in his role as chair and he is a major asset to the committee's success.. He is a strong advocate for the
elderly and disabled individuals living in long term care facilities and serves as a role model for the members of this
committee. Mr. Mason has attended 6 out of 7 business meetings during the past year and completes all quarterly
facility visits assigned to his group. Lastly Mr. Mason has directly participated in several advocacy projects that the
committee has sponsored and encourages other members to do the same.

Dr. Anthony John Vogt's current term to the Adult Care Home Community Advisory Committee will be expiring on
10/31/15. We are recommending that Dr. Vogt be reappointed for a second full term. Dr. Vogt has provided strong
leadership to the committee in his role as Vice Chair and is a major reason for the committee's success. He is a skillful
advocate, and has supported several projects that enhances resident quality of life,to include personalized music for
residents with dementia. He also serves as a facilitator for facility staff training on dementia care. Dr. Vogt has

group.

I have an interest in aging issues. This arise experience of my wife and I taking care of my
mother. Besides service on the Adult Care Home Advisory Committee and prior service on the
County Nursing Home Community Advisory Committee, I currently volunteer at Signature
(formerly Chapel Hill Rehab) Nursing Home in Chapel Hill.
Reasons for wanting to serve on this board:
The Advisory Board on Aging plays a key advisory role in the county on aging issues. Home
health care for the aging is very important, and neither the Adult Care Home nor the Nursing
Home committees address home health care for the aging in a major way. The County Advisory
Board on Aging can do this.
Conflict of Interest:
However, my wife and I are on the waiting list for the Carol Woods continuing Care Community.

Work Experience: I currently serve on the Human Relations Commission and wish to
continue serving on that Commission. I've been on the HRC for several years now and
feel I'm making a contribution. I've recently retired to Adjunct Professor status at the
School of Government of UNC. Because of this, I have more time to devote to volunteer
work and can serve on a second county committee. In the past, I served on the Nursing

Home Committee, found the experience to be rewarding, and feel that I made a valuable
contribution. I would like to serve on either the Adult Care Home Committee or return to
the Nursing Home Committee. My first choice is the Adcult Care Home committee. It has
more vacancies, and there is less regulation than for nursing homes; this makes service

The residents of adult care and nursing homes are too often neglected. Care in such
