Purpose of This Memorandum This Memorandum of Agreement outlines the relationship between United Way of the Greater Triangle ("United Way" or"UWGT"), who has agreed to provide $60,000 in funding to Orange County, a local political subdivision of the State of North Carolina, by and through the Orange County Department of Social Services("Orange County DSS")for the purpose of Orange County DSS serving as a Lead Agency to implement a new model of integrated service delivery that targets young people aging out of foster care and those still eligible for LINKS services. 2. Terms This Memorandum of Agreement shall take effect on June 1, 2015 and will go for one year, ending on May 30, 2016.The MOA may be renewed or extended beyond the effective date with the approval of both parties. 3. Partnership The partnership represented by this agreement is committed to working together with and on behalf of ALL young people aging out of foster care system and those who are eligible for LINKS services. United Way and the members of this collaborative will create a new integrated service delivery model designed to increase the stability of these young people as they move, independently into adulthood. This partnership will develop tools and strategies for implementing, describing and communicating the common elements of this overall partnership so that a)it can be adapted and implemented in other communities, and b)additional resources and funding can be secured. 4. Role of the Lead Agency As the Lead Agency the Orange County DSS is expected to make sure that the collaborative is effective and that all participants agree to their roles and responsibilities as outlined in Chart 1.The Lead Agency is responsible for ensuring the accurate pass through of financial assistance to the appropriate partners. Roles and Responsibilities Within the Partnership Partner Roles Orange County + Act as the lead agency within the collaboration Department of • Attend and participate in meetings with the Collaborative at a frequency Social Services determined appropriate by the partner agencies ($32,000) • Provide primary case management for all participants through the LINKS program. • Ensure each participant has access to eligible LINKS funding to promote the achievement of the seven outcomes recognized by the NC LINKS program. • Provide referrals of eligible young adults • Enroll all young adults (ages 18-21)with Building Futures program. • Provide employment services through Building Futures to assist young adults in reaching their employment goals. • Refer all young adults who meet criteria for the Rapid Rehousing program to assist in offering safe and stable housing. • Provide transportation for participants via agency and staff vehicles, if available and needed, during normal business hours • Maintain comprehensive case dictation for each participant and share information/data with all collaborative partners in regards to housing, income, and financial management outcomes. • Create, maintain,and submit all required financial documentation to the United Way of the Greater Triangle(UWGT)to receive reimbursement for all services provided by the Collaborative. • Collect financial documentation from Collaborative partners and reimburse each partner for services provided in a timely and accurate manner. • Generate and submit reports on behalf of the Collaborative as required by UWGT • Participate in all meetings with the UWGT to share data and further the goals of this Collaborative Durham Technical • Provide academic counsel,financial aid management, and career Community College resources to participants who are considering enrollment or who are ($4,000) enrolled at Durham Tech i • Provide referrals of eligible young adults • Attend and participate in meetings with the Collaborative at a frequency determined appropriate by the partner agencies • Maintain comprehensive case notes for each participant and share information/data with all collaborative partners in regards to educational outcomes. • Create and submit invoices and other required financial documentation to OCDSS for reimbursement. • Generate and submit reports on behalf of the Collaborative as required by UWGT • Participate in meetings with UWGT to share data and further the goals of this Collaborative Youth Villages • Provide transitional living services and trauma focused CBT to ($4,500) participants DocuSign Envelope ID:48959198-4FAC-490E-95C3-5A542B2E055D • Provide referrals of eligible young adults • Attend and participate in meetings with the Collaborative at a frequency determined appropriate by the partner agencies • Maintain comprehensive case notes for each participant and share information/data with all collaborative partners in regards to housing, income,financial management,and mental health outcomes • Create and submit invoices and other required financial documentation to OCDSS for reimbursement. + Generate and submit reports on behalf of the Collaborative as required by UWGT • Participate in meetings with UWGT to share data and further the goals of this Collaborative Life Skills • Provide independent living skill building,mental health services, and Foundation transitional housing assistance to participants ($17,000) • Provide referrals of eligible young adults Attend and participate in meetings with the Collaborative at a frequency determined appropriate by the partner agencies • Maintain comprehensive case notes for each participant and share information/data with all collaborative partners in regards to housing, income,financial management, and mental health outcomes • Create and submit invoices and other required financial documentation to OCDSS for reimbursement. • Generate and submit reports on behalf of the Collaborative as required by UWGT • Participate in meetings with UWGT to share data and further the goals of this Collaborative Community • Act as a resource to the Collaborative by connecting youth to Empowerment appropriate job openings with CEF partner employers. Fund ($2,500) • Work with employers to potentially offer paid on-the-job training utilizing grant funds. • Attend and participate in meetings with the Collaborative at least monthly 0 Maintain comprehensive case notes for each participant and share information/data with all collaborative partners in regards to employment. • Create and submit invoices and other required financial documentation to OCDSS for reimbursement. • Generate and submit reports on behalf of the Collaborative as required by UWGT • Participate in meetings with UWGT to share data and further the goals of this Collaborative 5. Performance Monitoring/Quality Improvement This project is a pilot of a new service delivery model for a specific target population. UWGT will be part of ongoing support to meet the challenges of the implementation of this new collaborative model. Efforts will be made to meet with the partners of the collaborative on a regular basis in order to promote communication and trust amongst the partners. DocuSign Envelope ID:48959198-4FAC-490E-95C3-5A542B2E055D The Orange County DSS and the partners of the collaborative agree to maintain client files and make them available to UWGT for monitoring, research or evaluation purposes.At a minimum,files should contain demographic information about the client, identified client goals/outcomes and progress on those outcomes, and documentation of expenses associated with each of these clients for education, employment and housing related activities. UWGT will support this collaborative by developing an integrated or shared data collection and reporting system that enables partners to target interventions and strategies in a way that ensures maximum impact and success. The partners will implement procedures to request permission to share data amongst themselves and with the UWGT. 6. Reimbursement UWGT will provide reimbursement based funding in the amount of$ 60,000 for the purposes of housing stabilization,educational and employment activities, and financial literacy and planning. The Orange County DSS shall submit by the 3rd day of each month a billing statement that consists of a tracking spreadsheet.The Orange County DSS will be reimbursed for salary cost of Life Skills Foundation staff time provided for each youth served during the previous month and any reimbursements requested for housing,employment, education, and transportation identified for the previous month.-UWGT will provide reimbursement by the last week of the month the submission was made. UWGT is not responsible for ensuring that the collaborative partners are receiving their appropriate or timely payment. 7. Amendments This Agreement may be amended at any time upon mutual written and signed agreements of Orange County DSS and UWGT 8. Termination of Agreement Either The Orange County DSS or UWGT may terminate this agreement by giving written notice of such termination to the other party at least thirty(30) days prior to the beginning of the fiscal year in which termination will take effect.
