PURPOSE: To amend the FY2007-08 contract with Personalized Patient Home Assistance Inc.
for In-Horne Aide Services to eligible adults.
BACKGROUND: The Department of Social Services is mandated to provide In-Home Aide
Services to older adults. In-Home Aide involves the provision of paraprofessional services that
assist functionally impaired older adults and/or their families with essential home management
and personal care and/or supervision to enable the older adult to remain at home as long as
possible.
The Department contracts with five providers to perform In-Home Aide Services for clients.
The Board of County Commissioners approved the original contract with Personalized Patient
Home Assistance, Inc. for $99,000 in June of 2007.
Social Services now estimates that $115,000 will be spent on in-home services provided by
Personalized Patient Home Assistance, Inc. during 2007-2008, and requests to amend the
contract accordingly. The contracts administered by Social. Services are impacted by client
choice, geographic Location and specific needs of elderly and disabled clients. This request
reflects the current spending pattern for in-home services.
FINANCIAL IMPACT: The total amount budgeted for In-Home Aide Services is sufficient to
cover costs incurred through this contract. No additional County dollars are required since
$16,000 in funds are being reallocated from other contracts that have been under spent.
RECOMMENDATION(S): The Manager recommends that the Board approve the contract
amendment with Personalized Patient Home Assistance, Inc. for Fiscal Year 2007-2008, and
authorize the Chair to sign the contract amendment.

SECTION I
Agency: Personalized Patient Home Assistance, Inc.
Program: In-Home Aide Services
Effective Period ofthe Contract: July 1, 2007 to June 30, 2008
This Contract Amendment amends the contract between the Orange County Department of Social Services (the
"County") and Personalized Patient Home Assistance, Inc. (the "Contractor"). As provided for under the terms of
the contract, the County and Contractor agree to amend the provision(s) indicated in Section II below.
SECTION II
Justification/Change to Contract:
The County shall pay the Contractor in the manner and in the amounts specified in the Contract Documents. The
total amount paid by the County to the Contractor under this contract shall not exceed $115,000. This amount
consists of $115,000 in Federal, State and County funds (CFDA # ), $0 (source of other funds if
applicable).
The total contract amount including any Contractor match shall not exceed $115,000.
SECTION III
All other terms and conditions set forth in the original contract shall remain in effect for the duration of the contract.
The contract specified above is amended by this Contract Amendment effective May 20, 2008.
