RE: Smart Start Contract or Amendment
The Department of Human Resources has approved the formation and operation
of the activities specified in the enclosed contract or amendment as set forth by
your County Smart Start Team. Your approved contract or amendment is being
returned to you fully approved.
Also enclosed is the current reimbursement form. We appreciate the time,
energy and effort expended in making your local efforts a reality. We look
forward to your progress and hope that you will contact the Division of Child
Development if we can be of any assistance to you. You may contact me at

AMENDMENT H
CONTRACT#0887
This agreement amends the contract between the North Carolina Department of Human Resources,
Division of Child Development,hereinafter referred to as the"Department' and the Orange County
Health Department,hereinafter referred to as the"Contractor". This Amendment is hereby
effective on September 30, 1994.
As provided for under the terms of this contract,the Department and the Contractor agree to
amend the following provisions:
1. Reference Part II. "Effective Period": The termination date of September 30, 1994 is
amended and changed to June 30, 1995.
2. Reference Part DC. "Payment for Services": The not to exceed amount is being
increased by$40,000.00 from$43,777.00 to$83,777.00.
3. Reference Part VIII. "Responsibilities of the Contractor": Attachment IA details the
activities that will be performed in accordance with the terms of this amended contract.
All other terms and conditions as set forth in the original contract document shall remain in effect
for the duration of this agreement.
IN WITNESS WHEREOF,the Department and the Contractor have executed this amendment in
triplicate originals,one to be retained by the Contractor,one to be retained by the Department and
one to be on file with Division accounting.

Orange County Health Department
The contractor will provide families and children,from birth to 5, having an
identified risk or who are already identified with any developmental delay,
social/emotional disorder or chronic illness,access to preventive,specialized and
support services. Contact will be made through the home,day care,telephone and
clinical visits at least quarterly; or more frequently if needed, to reassess needs and
do follow-ups or referrals. A social worker will also be hired,trained,and ready to
carry a case load by September 1, 1994.
