The agenda abstract dated April 19, 1994 recommends approval and that the Chair be authorized to sign. However, when I received the signed agreement., you had signed as Health Director. Please let me know if this is the same agreement and what I need to do legally about this agreement. BACKGROUND: The Health Department has received ongoing Medicaid reimbursement to fund a Maternity Care Coordinator whose caseload focuses on UNC Hospital patients. This agreement, which involves no exchange of funds, assures that the health department employee has space and access to UNC Hospital patients requiring her services. Because there is no cost associated with this agreement, the need to renew it was overlooked until recently. RECOMMENDATION(S) : The Manager recommends the agreement be approved and that the Chair be authorized to sign. 2 STATE OF NORTH CAROLINA UNCH # COUNTY OF ORANGE AGREEMENT BETWEEN The University of North Carolina Hospitals and Orange County THIS AGREEMENT, made and entered into this the 1st day of July, 1993, by and between University of North Carolina Hospitals (hereinafter referred to as the Hospital) for its Department of Social Work and Orange County (hereinafter referred to as the County) for its Health Department WITNESSETH: a WHEREAS, the Hospital and County wish to collaborate in addressing and correcting barriers that negatively affect pregnancy outcomes; and WHEREAS, the parties desire to execute this agreement to delineate this collaboration. NOW, THEREFORE, the parties hereby agree as follows: 1. The County agrees to make available the services of a Maternity Care Coordinator, without charge, to collaborate with Hospital staff in providing care to pregnant Orange County women. 2. The Hospital agrees to provide shared office space, accessibility to office equipment and telephone service, and liaison services from the Chief Clinical Social Worker, OB/GYN. 3 . The Maternity Care Coordinator shall provide Outreach, Recruitment, Assessment, Service Planning, Coordination & 3 Referral, Follow-up & Monitoring, Education & Counseling F ' activities with eligible women. These services will be coordinated with those provided by the Department of Social Work and other Hospital Staff. The person (or persons) whose services are to be provided pursuant to this Agreement is (are) for all purposes an employee of Orange County Health Department. However Social Workers employed by the County to fulfill this function must apply for and be certified as qualified to perform patient care activities in the Hospital.. The delineation of these privileges shall be set forth in an individual agreement between the Hospital and the Worker. This Agreement shall run for a period of one year, from the 1st day of July, 1993, to the 30th day of June, 1994 . It is the intention of the parties that this Agreement be reviewed annually and may be renewed. It may be terminated by either party upon 60 days written notice.
