Chapel Hill - 968-4501 Mebane - 227-2031 Durham - 688-7331 PURPOSE: For the Board of Commissioners to approve of a plan for the Health Department to participate in jointly funding a contract with the School of Public Health to conduct the Community Health Assessment of the Healthy Carolinians Task Force in collaboration with the UNC Hospitals, the School of Public Health and other community agencies. BACKGROUND: The Board of Commissioners, at their meeting of October 18, 1994, approved the establishment of the Orange County Healthy Carolinians Task Force as recommended by the Board of Health. At the Goals and Objectives Retreat on December 10, 1994, the Commissioners agreed to endorse the goal. A preliminary meeting of the selected task force members listed in Attachment 1 was held on August 28, 1995. There was unanimous agreement that the project should be undertaken. Implementation of the Healthy Carolinians Task Force has been difficult due to staffing shortages needed to get the project rolling. UNC Hospitals has agreed to invest funds in the project contingent upon the other partners doing so and the project resulting in a community health assessment which can be used in the hospitals Joint Commission on Hospitals Accreditation Review. The UNC School of Public Health has agreed to invest a similar amount if the project can result in a model that would be replicated elsewhere. The Health Department is asked to contribute $10,000 as a partner in the project. Attachments 2 and 3 outline the tasks to be accomplished by a coordinator hired by the School of Public Health and the proposed budget for the project. 2 The Health Director has identified $5, 000 from the Kellogg Mini-Grant that could be used for this purpose. Since the Board of Commissioners has adopted the Healthy Carolinians Task Force as a goal, the Board of Health at its meeting of August 24, 1995, agreed to request that the Board of Commissioners match this amount with an additional $5, 000 dollars. An outcome of the project would include a community health assessment that would result from broad community participation as well as thorough scientific and epidemiological methods. An additional outcome would be the establishment of a community wide partnership for health that would improve health conditions in Orange County. The Healthy Carolinians is not simply as assessment process, it also results in action plans to remedy the conditions identified in the assessment. THE PLAN 1) for the Health Department to participate in funding a contract with the UNC School of Public Health; 2) to authorize the development of a contract between the Health Department and the School of Public Health for a part-time project coordinator; 3) to allocate $5, 000 dollars from the Kellogg funded, Community Based Public Health Initiative; and 4) to allocate $5, 000 dollars from the Commissioners Contingency Fund. "Public Health is what we, as a society, do collectively to assure conditions in which people can be healthy" These words openned the summary of a report of the Institute of Medicine on the future of public health. In light of this the report recommended that health agencies at the local level assess the health status of its citizens, select problems of most concern and joint hands to reduce the negative effect of these conditions. The Orange County Board of Commissioners at the recommendation of the Board of Health, UNC Hospitals, the School of Public Health and Piedmont Health Services is about to establish a community task force to work toward achieving better health for our people. Opinion leaders from many walks of life: educators, policy-makers, administrators, providers, consumers, young and old alike are being invited to serve on the Orange County Healthy Carolinians Task Force. The members invited have been chosen because their opinions are valued, they have a genuine interest in the health and well-being of our people and have shown a commitment to being involved in community processes to find and apply solutions to local problems. Many community leaders have identified you as a person who has these characteristics. It is my privilage, therefore to invite you, on behalf of the Board of Commissioners and the Board of Health, to a meeting to pursue the formation of this task force. The meeting will be held on August 28, 1995, 12:00 - 1:30 pm, at the UNC-Hospitals Administration Office Building on Friday Drive beyond the Friday Center. An agenda for the meeting is enclosed as well as a list of those individuals and agency heads who have been invited to participate. Please indicate on the enclosed return card whether you will attend this meeting or designate someone to act on your behalf. CANCER AS THE SECOND LEADING CAUSE OF DEATH IN ORANGE COUNTY 10 The Healthy Carolinians 2000 Task Force Like many communities across the state and nation,yours is considering to launch a health improvement program.We believe that individuals and families are the key to your success;only through their commitment to live longer,healthier lives can a community achieve improved health.We also believe that people are more likely to work toward objectives that they have set for themselves.By developing a Healthy Carolinians 2000 Task Force,your com- munity will be able to estabish the community-based objectives necessary to improve its health. The Healthy Carolinians 2000 Task Force will: • Inform local leaders about health problems that can be improved by wellness promo- tion and disease prevention. • Create an ongoing,community-oriented process for priority setting and action plan- ning. • Select and tailor objectives from the list recommended by the state level Task Force. • Serve as the conduit for funding of cooperative health promotions. Healthy Carolinians 2000 activates the community to work in a concerted,collaborative approach It usually involves the following steps: • Organizing a community group. • Collecting and analyzing data. • Identifying health problems and setting priorities or targeting problems. • Identifying relevant community resources and barriers. • Developing an action plan. • Starting the action plan. • Assessing or evaluating progress. Several models for the community exist including traditional community development models, the Assessment Protocol of Excellence in Public Health (APEX PH) and the Planned Approach to Community Health (PATCH) (See Appendix 7 and 8).Remember that working with the community is an art and science requiring a functional community group that main- tains creativity and consensus building.
