THIS AGREEMENT, entered into this 19th day of December, 2014, by and between Orange
County ("County"), by and through the Orange County Arts Commission ("Arts Commission")
and Door to Door of UNC Health Care ("Grant Recipient").
WHEREAS, the Arts Commission is a countywide arts agency that ensures the growth of
local arts experiences by providing funding and support for diverse high quality programs and
projects.
WHEREAS, the Arts Commission administers the Spring and Fall Arts Grants Program
to provide economic opportunities to Orange County artists and non-profit organizations
coordinating arts projects benefiting the citizens of Orange County.
WHEREAS, the Spring Arts Grants Program is supported by Arts Commission with
funds from the Grassroots Program of the North Carolina Arts Council, an agency of the
Department of Cultural Resources and the National Endowment for the Arts.
WHEREAS, the Grant Recipient was selected by the Arts Commission to receive the
Spring Arts Grant through the procedures duly adopted by the Arts Commission; and
WHEREAS, both parties wish to promote and maintain the integrity and clarity of the
Grant Recipient's ideas and statements and delineate the Grant Recipient's obligations to the
County;
NOW, THEREFORE, the County and the Grant Recipient, for the consideration and
under the conditions hereinafter set forth, agree as follows:
1. Approved Grant Project. Grant funds are provided under this Agreement for the project
("Project") described in the Grant Recipient's Grant Application, attached hereto and
incorporated herein as Exhibit A.
2. Grant Award. Grant funds in an amount not to exceed one thousand five hundred dollars
($1,500) have been awarded by the Arts Commission to the Grant Recipient for the
approved Project. These funds must be matched at least dollar-for-dollar with cash from
the Grant Recipient's funds not derived from either Orange County appropriations or the
North Carolina Arts Council.
3. Contingency. This grant is contingent on North Carolina Arts Council Grassroots
Program funding to the Arts Commission. Final decisions on Grassroots Program funds
to the North Carolina Arts Council are subject to approval by the North Carolina General
Assembly.
4. Responsibilities of the Grant Recipient

A. Implementation. The Project implementation shall take place between January 1,
2015 and December 31, 2015. The Project shall be implemented in the manner
described in Exhibit A. Time is of the essence in the implementation of the
Project.
B. Modifications to project. Grant Recipient shall present major changes to the
original proposal, including but not limited to changes in design, implementation
schedule and increase in Project budget by more than 10%, in writing to the Arts
Commission for approval. Arts Commission shall approve or disapprove Grant
Recipient's request for modifications within seven (7) days of receipt thereof.
Consent to project modifications shall not be unreasonably withheld. Arts
Commission's decisions shall be in writing. In the event the Arts Commission
fails to approve or disapprove the requested modifications within the time stated
herein,the modifications shall be deemed approved.
C. Non—assignm The grant cannot be assigned or transferred to a different project
or artist without prior written approval of the Arts Commission.
D. Records. Grant Recipient must maintain records and other evidence pertaining
to costs incurred and revenues acquired during the Project implementation. Grant
Recipient shall maintain financial records pertaining to the Project for three (3)
years from the date of this Agreement, in case of audit.
E. Use of Funds. Grant Recipient shall not use the grant funds to repay outstanding
taxes or other debts of any nature. The grants shall be utilized solely for the
purpose of implementing the Project and Project related expenses.
F. Final Report. Grant Recipient shall submit to the Arts Commission a Final Report
on January 15, 2016 . The Final Report shall be in the format required by
the North Carolina Arts Council. Failure to submit the Final Report or late
submission of the Final Report, for any reason, shall be grounds for denial and/or
cancellation of grant funds for the Project and future grant proposals.
5. Responsibilities of the Arts Commission
A. Pam. The Arts Commission shall release the grant funds in the amount of
$1,500 to the Grant Recipient.
6. Publicity
A. Acknowledgement. Credit must be given to the Orange County Arts
Commission using the Arts Commission logo and if State funds are received
to the North Carolina Grassroots Program in brochures, news releases,
programs, and all other printed and online publications. Copies of such printed
materials must accompany the evaluation report. When no printed information is
used, verbal credit shall be given. In addition, all print and promotional material
must include the following credit line (check all that apply):

i. x County Funds Only. "This program is supported by the Orange County
Arts Commission. "
ii. North Carolina Arts Council Funding. "This program is supported by
the Orange County Arts Commission with funds from the Grassroots Program
of the North Carolina Arts Council, a state agency. "
B. The Grant Recipient must enter all events, exhibits and performances open to the
public and related to this grant award on the ExploreChapelHillARTS.com
website and display ExploreChapelHillARTS.com publicity materials
(postcards, brochures, etc.) prominently and consistently at their venue or
performance space. In addition, the Grant Recipient must add a link to the
ExploreChapelHillARTS.com (ExploreChapelHillARTS.com logo preferred)
to their main webpage. The Grant Recipient shall'contact the Orange County Arts
Commission for replacement ExploreChapelHillARTS.com publicity materials.
7. Indemnity. The Grant Recipient agrees to indemnify and hold harmless the County, the
Arts Commission and the North Carolina Arts Council and their officers and employees
from any and all claims or actual injury, damage or loss to a person, or real or personal
property that results from or is in any way connected to the use of the grant funds.
8. Independent Contractor. The Grant Recipient is an independent contractor and not an
employee, agent or other representative of the County and/or the Arts Commission.
Nothing contained in this Agreement shall be construed to create the relationship of
principal and agent, or employer and employee, between Grant Recipient and Arts
Commission or Orange County Government. Grant Recipient understands and agrees
that he/she is not authorized to incur any expenses or any liability whatsoever on behalf
of the County or the Arts Commission and has no authority, expressed or implied, to
obligate or make representations on behalf of the County or the Arts Commission. In
addition, Grant Recipient agrees to maintain his or her own worker's compensation
insurance for himself/herself and his or her employees for the duration of the art work's
installation.
9. Termination
A. In the event of any of the circumstances set forth below(hereinafter referred to as
"default"),the Arts Commission may immediately terminate this Agreement:
(i) Any fraudulent representation in any verification required to obtain
payment under this Agreement or other dishonesty on a material matter
relating to the performance of services under this Agreement.
(ii) Nonperformance, incomplete service or performance, or failure to
satisfactorily perform any part of the Project or to comply with any
provision of this Agreement, as determined by the Arts Commission in its
sole discretion.

(iii) Failure to adhere to the terms of applicable county, state or federal laws,
regulations, or stated public policy.
B. In the event of default by the Grant Recipient, the Arts Commission has the
discretion to elect to terminate this Agreement, in whole or in part and/or require
the Grant Recipient to repay the grant funds within seven (7) from written notice
of default or allow the grant recipient the opportunity to cure the default. This
clause shall not be interpreted to limit the County's or the Arts Commission's
remedies in law or in equity.
10. Miscellaneous.
A. Entire Agreement. This Agreement and the attachments contain the final and
entire understanding of the parties with respect to the subject matter of this
Agreement. All modifications and amendments to this Agreement shall be in
writing. This Agreement shall be binding upon the parties, their heirs, successors
and assigns.
B. Governing Law. This Agreement shall be interpreted in accordance with the laws
of the State of North Carolina and the Orange County.
IN WITNESS WHEREOF, the Orange County and the Grant Recipient have signed this Grant
Agreement, effective on the last date this Agreement is signed by both parties as indicated by the
dates set forth under signatures below.

Summary Statement for Proposed Uses of Arts Program Grant :
Funds especially from the Orange County Arts Commission honor artists who bring their
performing, visual and literary arts to the Psychiatric and Physically injured and
diseased patients. This year, DooR to DooR is requesting funds to bring in 1 literary, 2
Visual Artists, 4 performing, and 5 musicians for the Psychiatric patients and the
Oncology Patients in Chemotherapy, in Bone Marrow Unit and in Oncology patient
rooms. Additionally, working with a visual artist, I am putting together a card that will
carry the beauty of a DooR on the cover, and a piece of writing within, that is written by
participants of the Writing for Caregivers program DooR to DooR co-directs with the
Chaplain Heidi Gessner. This grant from DooR to DooR is requesting funds for these
cards ( a sample is enclosed) and for a booklet of writings for patients and a separate
booklet of writings for their caregivers.

Organizational Profile- Mission and History
Our mission,since 1991,when The DooR to DooR program of UNC Health Care was
founded in association with UNC Hospital Administrators and Physicians, is to bring life
and joy and solace to the patients,their families and the care giving staff by integrating into
the hospital environment the gifts of our community's finest artists. Chosen for their
artistic excellence and communication skills,and then escorted directly to the patients for
visits running from 5 - 20 minutes,well over 2,100 artists have visited well over 100,000
hospital denizens. UNC Hospitals has 804 in-patient beds.
DooR to DooR's excellent performing, (piano, guitar,mandolin,kora,violin, accordion, flute,
percussion,harp,hammer dulcimer, ukulele,lyre,bass,harmonica and a cappella song)
literary, (poet, actress, story teller,) and visual artist, (painter,water colorist,
photographer, caricaturist, cartoonist, origami and collage maker,) bring their arts into the
patients' rooms, as well as Dialysis,the Burn Center,staff rooms, Oncology, Bone Marrow
and Chemotherapy Clinics, Surgery, Emergency, Pediatric Waiting Areas and lobbies,which
become mini-theaters and classrooms. Those visits evoke words like these from patients,
staff and our artists:

without a single visitor,but having Joy and Teresa come to see me lifted my spirits so so so

well.)"

"What a joy to have this group here today! They provided a lighthearted delightful

Artists:
"The door to door program is the most rewarding work I have ever done as a performer.
Singing in the cancer patient's room is bringing a direct message of joy,beauty and
humanity to someone who desperately needs it,perhaps more than anyone in the world.
The results are almost always immediate and life changing. "

Current Programs and Services-a brief List
1. Artists escorted to patient rooms,waiting areas, clinics and units, approximately 10
visits a month.
2. To teach at the UNC child and adolescent, Crisis, Psychosis, Eating Disorders and
Geriatric Psychiatric Units: 45 Artists visit usually on Saturday or Sunday.
3. ArtHeels, called "a key to DooR to DooR" during the school year, UNC art majors and
others,volunteer to visit mostly child patients with mostly visual arts and clowning and
some music. The first year there were 15 volunteers, now in its 4th year there are over 60
volunteers extremely well organized and trained by their student coordinator. They visit
patients, mostly pediatric and mostly with visual arts, 6 days a week.
4. DooR to DooR website maintained, photographs added as often as possible taken by
director Joy Javits and sometimes by others. The designer of the website is Andrea
Ferguson, of Ride Again Productions, a supporter! of the program. -She donated the design
and a year of webhosting. She charged a nominal fee to maintain and update this excellent
website: www.doortodoornc.com She had been donating her service again!
S. Maintain the 5! Pianos: one in the lobby of the NC Children's Hospital and another in
the Dining Area for the Rehabilitation Patients,a beautiful baby grand, and another one in
the waiting area for chemotherapy patients, one in the Unit of the Psychosis Adult Patient
Unit and recently donated a paino in the Geriatric Psychiatric Patient Unit. Pianists play at .
all of them as organized by DooR to DooR and tuning and cleaning are coordinated by DooR
to DooR.
6. A Grant received from LiveSTRONG has continued to inspire this Coordinator to work
.with the patients directly and to make that artistic work interactive. For example Gabriel
Pelli brought his fiddle to play and also brought a small guitar which he tuned with an open
string and that allowed several patients who said, "I'm not at all musical"to have a go at
playing the guitar. It also allowed a Burn patient/musician to join him and play with
Gabriel for several other burn patients. Natalie Gilbert teaches patients in Psychosis and
Geriatrics, and with the keyboard purchased so we can go anywhere,to adolescents, Crisis
and child psychiatric patients to play the piano and play with each other. Billy Stewart
gives mini-guitar lessons to patients. Eric Martin brought the Ukulele DooR to DooR
purchased to children to play as he played his Ukulele. Visual artists, Rebecca Rousseau
and Liz Bouyeau encourage patients to make collages. Braima Moiwai to play the drums.
Impromptu Players to dance and improvise.
Proposed Uses of Grant Funds
This year, DooR to DooR is requesting funds to bring in 4 writers, 4 actors, and 4
musicians for the Psychiatric patients and the Oncology Patients in Chemotherapy, in
Bone Marrow Unit and in Oncology patient rooms.

BUDGET- 2014- 2015 Fall
Funding for Artists: I will bring each of these excellent artists and performers listed below,
to the Psychiatric, Child,Adolescent, Crisis, Psychosis, Perinatal, Eating Disorders or
Geriatric Units. And to the Oncology Patients over the two to three hours they come to
share their gifts. With Psychiatry,these are group events with usually about 10 people.
With the Oncology visits those are one on one in their hospital rooms. The staff and their
visitors,family or friends, are delighted to have something to share with the patient!
Funding for the Ca4Mini-Books: $300 for printing of 250 card gifts and 100 mini-books.
Ongoing My goal is to have An at least four artists visiting each week of the year.
I am working on fund raising from individuals. My family and friends give much of the donations
I receive from individuals. Sometimes patients, their families, administration of the hospital and
new members of the UNC community contribute too!

Back in my music room, I released this pleasant moment of nostalgia about
my instrument, checked it's tuning, and put it into its case. As a musician, I
love this time: when you are preparing your tools of the trade for their
rendering into people's experiences. Sometimes I pause for a second and
think: yes, kora, you are going into a case now, so compose yourself,
because soon you will be singing, gliding, sailing the notes into people's
ears,into the folds of their skin, and just maybe —into their heartspace.
That morning I was headed to UNC Hospitals for the second time to play for
patients. The week before, I was a patient.
I thought about this as I walked into the hospital to find a bustling holiday
market going on in the lobby. Cutting straight through the hubbub to meet
me was Joy Javits, the organizer of DooR To DooR, a non-profit that works
to bring the arts into UNC Hospitals for patients and staff. Joy is a lovely
person that seems to float around ever-so-slightly, like Tinkerbell, ultra-
aware of what's happening around her. She's very mindful of people's
boundaries and knows how to communicate; she's like a patient-whisperer.
She put together DooR To DooR in 1993, which brings in over 200 artists
and performers each year. Joy delicately facilitates their contact with
patients and staff, setting them up with stage performances or going room to
room to perform for patients......
Joy and I decided that we'd go over to the cancer hospital, 3rd floor I
believe. Bringing a kora and its music into the hospital is like a scaled-down
UFO landing —everyone is intrigued and captivated by this foreign thing. I
played for 3 patients in a row: a middle-aged guy who was having his vitals
taken, a fella in his 80s from Raleigh who was eating lunch with a shaky
hand, and a mother with her daughter who didn't seem sick at all.
While Joy was out finding my next patient, I was playing quietly in the
hallway when I was approached by a nurse. She said "I know someone who
you might could play for. He's 24 and has been in here a week. He has a
very bad prognosis and refuses to talk to anyone. He'll probably say no, but
I'll ask. I did his intake, so I have an `in' with him." She came back the same
time as Joy, and said that the 24-year-old had agreed. We all gave a tiny
cheer and then went over to his room together. I sat in the doorway to play
because he was under a `Contact Warning', meaning he could easily catch
something from me.

He was on the other side of the room,focused on his laptop and phone and other devices,
and he welcomed me with no expression. I greeted him and began to play a new song of
mine called Mes Amis du Kalaban Koura. It has a nice,uplifting, churning spirit to it,and
I focused on sharing that spirit with him. Often when I,play kora,I don't look directly
into people; that's the job of my notes.But this time I looked at him and smiled,and
noticed he was really interested in what I was doing.After I was done,he asked me lots
of questions about the kora and where it comes from and what the heck I was doing
playing it so well.I played another song,a traditional piece from Mali called Kulanjan
(not originally a kora piece but adapted to kora) and handed him a complimentary CD
and wished him well.
A few minutes later his nurse walked up to me in the hallway. She had tears in her eyes.
She looked at me and said,"you don't know what you just did."
I think she said a few other things,but I never got past the first part.And I don't think I
ever will. Because it's moments like this that urge me to feel like my chosen (by me or
the universe?) profession is the right thing to be doing.That the calling I feel to be a
musician is exactly that,a calling.And the universe wins when people answer their
calling,right? My bank account begs to differ,but my heart thumps harder and I am more
happy every second that I spend sending a note from me to you.
So if it's part of my job to bring a strange instrument from a distant place into totally new
territory,I'm game.

A Mini Lesson Regarding Confidentiality and Safety
For PERFORMERS at UNC Hospital
Thank you for sharing your compassionate heart and your professional artistry
with the patients, with their families and with the care giving staff of the
hospital.
1. PREPARATION; When you think about what you would like to bring to the
hospital patients, consider offering something with humor, something
sophisticated, something beautiful and something strong, something to engage
their imagination, taking them to a healing place, something soothing to ease
their pain perhaps induce sleep and something remind them that they are part
of the world outside the hospital.
Alternatively, you may have one good moment of art to bring. Repetition can
be grand. Jacki Shelton Green once came with one poem about her
grandmother which she recited 23 times and gave each one of the patients a
book mark with that poem printed on it A gift for the patients of words or
images can be wonderful, if not too expensive.
2. WHEN & WHERE: I ask that you commit 2 -3 hours of time to performing.
We will visit patients in their rooms, families in waiting areas and public
spaces and care giving staff everywhere they are.
3. ATTIRE: Please dress in clean well fitting clothes that cover most of your
body. Please do not wear black; although the uniform of performers, it has
another connotation in the hospital. Please wash yourself and your clothes
before and after your visit to the hospital. Scents are unpleasant for some
patients, please do not wear perfume or after shave.
4. CONFIDENTIALITY: I will accompany you everywhere we go. I will do
most of the handing out of cards etc. Touching the patients is wonderful with
their permission, i.e. shaking hands or hugging. Asking their name is not
permitted, asking "What may I call you?" is. HIPAA rules are for
confidentiality. We all want that. Whatever you see, whomever you meet in
the hospital is for you to keep confidential. If you describe an hospital
experience to a friend or colleague, leave out of your story any name or
identifying feature of the patient.
5. SAFETY: Before entering a patient area the first time, please wash your
hands in the sink with soap, and very little water to begin, singing Happy

Birthday twice through, around 20 seconds, then rinse. After that, in
between visits to EACH patient, use the antibacterial foam, on the wall,
(Some patients will want to shake your hand or might take a card from you or
may want to give you a hug.) For most patients we can go into their room.
There are other signs requiring vigilance on our part "Droplets" and "Air
Borne." We will not enter these rooms. For rooms with the sign CONTACT on
the door, we may stand in the hall just outside the opened door.
6. PROCEDURE: I will ask the Charge Nurse's permission to come into each
area we visit. Then I will tap on the patient's door softly, not opening the
door if they do not respond. I don't want to wake anyone! I will then ask if
they would "enjoy a visit from Charles Pettee who is playing the mandolin and
singing mountain music," for example. I will be specific so they can decide if
they would.like a visit. If they say no I will wish them well and look for
another room. This "No" may be the only choice they get to make that day
and is empowering. Often I will go the open doors first. I announce the name
of some of our excellent supporters. At the end of the visit I will give them a
card with the program and the performer's name on it.
7. AWARENESS: The musician needs to be aware of the volume of the
sounds. Most patients are fearful, drugged or both. Some have headaches,
some are hard of hearing. We establish all we can before you begin.
8. SENSITIVITY: You may see some faces and bodies and sores and blood
and severed appendages and vomit and spittle and disfiguration and bandages
and IV poles and wheelchairs and walkers and hard things to experience.
Thank you for the compassion and empathy that led you to come share your
music, poetry, visual art with all those who are ill and those who are taking
care of them.
9. YOUR HEALTH: Please call me as late as a few minutes before a scheduled
event if you are ill, or someone in your near family is ill, (you could have
something in or on your body or clothes from them.) It would be best if we
rescheduled.
