Subject: Reappointments to the Aging Board
Good Afternoon,
The Aging Board would like to recommend that Donna Prather and Keith Cook be reappointed for a second term. With
these reappointments it means that we have one vacancy on the board. The September board meeting is going to be a
joint meeting with the Nursing Nome and Adult Care CACs to review a long -term care study that was completed this
summer by one of our interns. We know that we have several applicants for the one vacancy. Those persons will be
invited to our October board meeting.
Let me know if you have any questions and thanks for your assistance in getting Donna and Keith reappointed.
Thanks,
Janice Tyler

Vice Chair of Orange County Adult Care Home Community Advisory Committee.
Boards /Commissions applied for:
Advisory Board on Aging
Background, education and experience relevant to this board:
I have an interest in aging issues. This arise experience of my wife and I taking care of my
mother. Besides service on the Adult Care Home Advisory Committee and prior service on the
County Nursing Home Community Advisory Committee, I currently volunteer at Signature
(formerly Chapel Hill Rehab) Nursing Home in Chapel Hill.
Reasons for wanting to serve on this board:
The Advisory Board on Aging plays a key advisory role in the county on aging issues. Home
health care for the aging is very important, and neither the Adult Care Home nor the Nursing
Home committees address home health care for the aging in a major way. The County Advisory
Board on Aging can do this.
Conflict of Interest:
However, my wife and I are on the waiting list for the Carol Woods continuing Care Community.
Supplemental Questions:
Work Experience: I currently serve on the Human Relations Commission and wish to
continue serving on that Commission. I've been on the HRC for several years now and
feel I'm making a contribution. I've recently retired to Adjunct Professor status at the
School of Government of UNC. Because of this, I have more time to devote to volunteer
work and can serve on a second county committee. In the past, I served on the Nursing
Page 2 of 2
Anthony John Vogt
Home Committee, found the experience to be rewarding, and feel that I made a valuable
contribution. I would like to serve on either the Adult Care Home Committee or return to
the Nursing Home Committee. My first choice is the Adcult Care Home committee. It has
more vacancies, and there is less regulation than for nursing homes; this makes service
on the adult care home committee to be potentially useful. 08/20/2012 UPDATE: Faculty,
School of Government, UNC -CH 1973 - 2009; Budget Analyst, State Budget Office,
Wisconsin 1968 -1972
Volunteer Experience: Nursing Home Community Advisory Committee (2000- 2006).
Served about five years, including as chair of the committee for one year or so. Human
Relations Commission: (2006- 2012). UNC Health Care, ACC, (2011- 2012); Kindred
Nursing Home (2011 -2012)
Education: Ph.D., Cornell University, 1973; Court work at Cornell 1968 -1970
MPIA, University of Pittsburgh, 1965
B. S., Georgetown University, 1963
Other Comments:
The residents of adult care and nursing homes are too often neglected. Care in such
facilities too often falls short of acceptable standards. Since leaving the Nursing Home
Advisory Committee several years ago, I have continued to visit one or two friends.
08/20/2012: 1 have interest that is strong in care for elderly. Served as Chair of Nursing
Home Committee for a year or two. STAFF COMMENTS: Reapplied 7/3/2005 for HRC
Adult Care Home Community Advisory Committee and OPC Area Board. App. 1/12/2000
for Innovation and Efficiency Committee; REAOPPLIED FOR ADULT CARE COME
COMMUNITY ADVISORY COMMITTEE 08/20/2012. ADDRESS VERIFICATION: 713

Advisory Board on Aging
I am energized by solving problems and taking on challenges. With over 1
person turning 50 every second due to the dramatic achievements in public
health, Orange County is poised to see a 67% increase in persons over 65
and a 43% increase in persons over 85 in the next 10 years. This trend
continues through 2030 when persons over 65 will again increase by 42%
and over 85 by 70 %. Orange county is a leader in serving its senior
population and I want to contribute to preparing and meeting these needs
throughout the next 20 years,
Although I worked as a chemical engineer for 15 years, I recently returned
to school to get a BA in Multidisciplinary studies, with an emphasis on social
sciences and the Socio- Ecological Model which shows that the health of an
individual is based not just on the person but withing the greater context of
his relationships, his community and his society. Because of my interest in
the aging process, I am currently completing a Masters in Gerontology from
the University of Southern California. Through this program I have been
able to receive the training and tools that will aid me in pursuing my goal of
improving the disparity in the aging experience between the different
economic classes. The research area where I am basing my concentration
consists of early depression intervention for the home caregiver. Many
home caregivers are thrust into their duties with little to no training and as
more residents, especially in the rural areas, are deciding to age in place
there will be a need for increased programs to address the adult child or
spousal caregiver, many of whom are over 50.
I am excited to be beginning this new phase of my life as I turn 50, and
consider myself to be a life -long learner who believes that we are all part of
something larger and there is purpose to all our lives. My belief system
influences all I do and I value responsibility and high ethical standards. I
Page 2 of 2
believe that the strategic methods of problem solving I utilized as an
engineer allows me to find and evaluate alternatives and seek innovative
ways to get things done. I believe in community involvement as I was a
member of the School Improvement Team (SIT) committee for McDougle
Middle School for 2 years, worked as a volunteer at the Chapel Hill Museum
for 3 years, and have been a member of the MAC - Mandarin Advisory
Council for the dual language program in CHCCS since it began and my
daughter has been part of the program for the past 8 years. As my children
are growing up and moving on to new goals, 1 feel that I too must focus my
time and energies on the future.
I appreciate your time in considering me for this position.
Nursing Homo Community Advisory Committee
I am graduating with my Masters of Arts in Gerontology in May 2014 from
the University of Southern California. My BS is multi disciplinary in
psychology and public health. My concentration of study is in environmental
interventions to decrease agitation in persons with dementia. I currently
volunteer at Galloway Ridge and care part time for my elderly parents who I
relocated to Chatham County in 2008.

Orange County Parks and Recreation Council
Background, education and experience relevant to this board:
Teri J. Driscoll
I spent 18 years as a stay -at -home Mom many years ago raising my children. At that time I was
involved in three schools that my children were attending and always volunteered for the
Parent/Teacher Organizations. I was also an officer for many years, serving as Pres,. VP and
Sec. for the PTO. I was involved with the local Township Recreation Board where I lived also.
Helped organize many soccer teams and schedules for the County where I resided.
Reasons for wanting to serve on this board:
I have lots of spare time and interestingly, I just had the opportunity to review your brochure that
has a list of all the Recreation Department activities for this upcoming fall. I thought there were
lots of good events listed there and since I have 2 grandchildren who live close by and visit me
often, I thought that maybe I could help this committee out with planning events. I also know
that there are vacancies on this committee.
Conflict of Interest:
Advisory Board on Aging
Background, education and experience relevant to this board:
I am a former Office Manager of an Assisted Living Home. I served on the Adult Home Care
Community Advisory Board. I am currently on the Nursing Home Community Advisory Board
and am Secretary. I attend all leadership conferences held by Triangle J. I was just to one
yesterday. I am very much concerned about the care of adults in the nursing homes. I have
always done all of my site visits and prepare the reports for my committee.
Reasons for wanting to serve on this board:
See above answers. I had the opportunity to attend a meeting of the Department of Aging this
month and found it very interesting. It would relate to my service on the Nursing Home CAC and
I feel that my attending the leadership classes that I do and visiting the facilities and it would be
another committee that I could contribute to. You may contact Ed Flowers, Chair of the Nursing
Home Advisory Board; Charlotte Terwilliger, Ombudsman or Mary Fraser, Dept. of Aging for
references. I am certain that they would recommend me for the Board. I have applied for other
Board vacancies but would like to make my application to this Board a priority. I am retired and
have plenty of time to give back.

As a person who is aging I recognize the necessity of recognizing how this process effects
people physically and mentally. I have worked as a geriatric social worker and am a long time
believer in the connection of maintaining physical and mental activities as well as connection to
the larger community to help people age with integrity and dignity. Recently retiring as a high
school teacher, I clearly see the importance of maintaining ties to a larger community to lessen
the isolation created with retirement. I would like the opportunity to bring my ideas to a larger
group in order to be more inclusive of the aging population.
