GENERAL INFORMATION This document was developed to provide a uniform format for handling contract changes that affect Contract Price or Contract Times. Changes that have been initiated by a Work Change Directive must be incorporated into a subsequent Change Order if they affect Price or Times. Changes that affect Contract Price or Contract Times should be promptly covered by a Change Order. The practice of accumulating Change Orders to reduce the administrative burden may lead to unnecessary disputes. If Milestones have been listed in the Agreement, any effect of a Change Order thereon should be addressed. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Times,a Field Order should be used. B. COMPLETING THE CHANGE ORDER FORM Engineer normally initiates the form, including a description of the changes involved and attachments based upon documents and proposals submitted by Contractor,or requests from Owner,or both. Once Engineer has completed and signed the form, all copies should be sent to Owner or Contractor for approval, depending on whether the Change Order is a true order to the Contractor or the formalization of a negotiated agreement for a previously performed change. After approval by one contracting party, all copies should be sent to the other party for approval. Engineer should make distribution of executed copies after approval by both parties. If a change only applies to price or to times,cross out the part of the tabulation that does not apply. Hammersley, County Manager Through: Craig Benedict, Planning Director From: Kevin Lindley, PE, Staff Engineer pp FiA�' Date: July 28, 2014 RE: Buckhorn Mebane Phase 2 Utility Project Change Order#5 This change order is to correct the original contract price to match the original project scope. When the project contract was signed,the contract price was determined by a schedule of pay items and projected quantities. This pay schedule should have included quantities and line items necessary to complete a short section(approximately 700 linear feet) of 8" sewer line, designated on the plans as line S-2. These items were inadvertently left off the schedule of pay items and quantities, even though the written description of the project on the contract and the project plans issued for construction included line S-2. This change order also includes the installation of a service connection for Mr. Don Collins on property with PIN 9834569034. Mr. Collins paid for this service connection in advance as part of his easement agreement with the County, but the service connection line item was not part of the bid documents, because the agreement with Mr. Collins was not finalized until after bids were received. This is the only service connection to be installed as part of this construction project. The cost of this construction work will be covered in project budget due to significant savings realized by modifying the method of boring under several streams along the project route. The contract was bid under the assumption that the streams would have to be directionally bored, but field conditions were such that a much less costly bore and jack method could be used. The cost of this change order has been summarized below, along with the summary for the first four change orders. Based on the original schedule the allowed substantial completion date would have been 4/16/2014, or 365 days. CO#1 added an additional 57 days due to delays in obtaining easements and the time to complete the extra work. This extended the contractually allowed substantial completion date to 6/12/2014. CO#2 added an additional 8 days due to inclement weather, which pushed the allowed substantial completion date out to 6/20/2014. CO#3 requests an additional 76 days due to delays in easement acquisition on the Rohl property along line S-1 and delays associated with realignment of the beginning section of line S-1. Adding these 76 days would reset the allowed substantial completion date to 9/4/2014. CO#4 adds no additional time. CO#S adds no additional time. Please call or email with any questions.
