CONTRACT AMENDMENT
ORANGE COUNTY
THIS CONTRACT AMENDMENT is made and entered into this__/__day , :?014 by and between
ORANGE COUNTY(hereinafter referred to as"County")and LEANNE B.P LAI NIS (hereinafter re tc rred to as
"Provider").
WITNESSETH:
THAT WHEREAS,the County and Provider entered into a contract dated July 1, 2013, for the provision of transcribing
services to the County,(hereinafter the"Original Agreement"); and
WHEREAS, the County and Provider desire to extend the Original Agreement while keeping in effect all terms and
conditions of the Original Agreement not inconsistent with the terms and conditions set forth below.
NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the parties a,ree to
amend the Original Agreement as follows:
1. The Term of the Original Agreement is hereby extended through June 30,2015.
2. Section 1, "Payment" is amended to read as follows: The County agrees to pay at the rates speL i fled for
Services satisfactorily performed in accord with this Agreement. The amount to be paid by the County shall
not exceed thirteen thousand dollars ($13,000.00), at $85/per recorded meeting hour of Board of County
Commisisoners' meetings, per fiscal year. Payment shall be made within thirty (30) days of an invoice
properly submitted to County. Should Provider fail to perform its duties under the terms of this Agreement,
County may, without fault or penalty, withhold any payment associated with the work to be performed until
such time as said work is completed.
3. Except for the changes made herein,the Original Agreement shall remain in full force and effect to i he extent
it is not inconsistent with this Amendment. In the event that there is a conflict between the Original
Agreement and this Amendment,this Amendment shall control.
