The first of these two requests asked for 8 extra days due to inclement weather, as shown on the supporting documentation. The second request was for extra cost required to resurvey/layout the sewer line across the Rohl property north of West Ten Road. This was a County-initiated shift in sewer alignment made necessary based on the results of the Cultural and Archaelogical Survey performed pursuant to County Policy. This cost can be covered by the existing project budget. It will be paid out of a portion of the budget set aside as an easement resolution contingency. The costs of this change order have been summarized below, along with the summary for Change Order#1. Going forward, new change orders can be added to the summary so that all the change order information will be available in one document. Original Contract Price: $4,016,545.74 - Original rock contingency for water line: $ 67,500.00 (included) CO#1 $ 15,328.00 . . z - Extra work to avoid an unmarked 16" force main - Reroute a 4" water main - Delays due to easement dispute - Revised rock contingency for water line: $ 52,172.00 Net Contract Price Change for CO#1: $ 0.00 CO#2 $ 2,242.50 - Cost due to resurveying/relayout of sewer S-1 - Done to avoid a potentially significant cultural/archaeological site on the Rohl property Revised Contract Price after Change Orders: $49,0189788.24 SCHEDULE The project start date was 4/16/2013. Based on the original schedule the allowed substantial completion date would have been 4/16/2014, or 365 days. C0#1 added an additional 57 days due to delays in obtaining easements and the time to complete the extra work. This extended the contractually allowed substantial completion date to 6/12/2014. CO#2 requests an additional .8 days due to inclement weather, which would push the allowed substantial completion date out to 6/20/2014. Please call or email with any questions. Change Order Instructions A. GENERAL INFORMATION This document was developed to provide a uniform format for handling contract changes that affect Contract Price or Contract Times. Changes that have been initiated by a Work Change Directive must be incorporated into a subsequent Change Order if they affect Price or Times. Changes that affect Contract Price or Contract Times should be promptly covered by a Change Order. The practice of accumulating Change Orders to reduce the administrative burden may lead to unnecessary disputes. If Milestones have been listed in the Agreement, any effect of a Change Order thereon should be addressed. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Times, a Field Order should be used. B. COMPLETING THE CHANGE ORDER FORM Engineer normally initiates the form, including a description of the changes involved and attachments based upon documents and proposals submitted by Contractor, or requests from Owner, or both. Once Engineer has completed and signed the form, all copies should be sent to Owner or Contractor for approval, depending on whether the Change Order is a true order to the Contractor or the formalization of a negotiated agreement for a previously performed change. After approval by one contracting party, all copies should be sent to the other party for approval. Engineer should make distribution of executed copies after approval by both parties. If a change only applies to price or to times, cross out the part of the tabulation that does not apply.
