HomeMy WebLinkAbout2013-522 Arts - Door to Door of UNC Health Care a© C a ORANGE COUNTY ARTS COMMISSION Z v a a c a u r r TS FALL, 2013 GRANT AGREEMENT \ CnIlItAINSION U n c THIS AGREEMENT, entered into this 12th day of December, 2013, by and between Orange County ("County"), by and through the Orange County Arts Commission ("Arts Commission") and Door to Door of UNC Health Care("Grant Recipient"). WHEREAS, the Arts Commission is a countywide arts agency that ensures the growth of local arts experiences by providing funding and support for diverse high quality programs and projects. WHEREAS, the Arts Commission administers the Spring and Fall Arts Grants Program to provide economic opportunities to Orange County artists and non-profit organizations coordinating arts projects benefiting the citizens of Orange County. WHEREAS, the Spring Arts Grants Program is supported by Arts Commission with funds from the Grassroots Program of the North Carolina Arts Council, an agency of the Department of Cultural Resources and the National Endowment for the Arts. WHEREAS, the Grant Recipient was selected by the Arts Commission to receive the Spring Arts Grant through the procedures duly adopted by the Arts Commission; and WHEREAS, both parties wish to promote and maintain the integrity and clarity of the Grant Recipient's ideas and statements and delineate the Grant Recipient's obligations to the County; NOW, THEREFORE, the County and the Grant Recipient, for the consideration and under the conditions hereinafter set forth, agree as follows: 1. Approved Grant Project. Grant funds are provided under this Agreement for the project ("Project") described in the Grant Recipient's Grant Application, attached hereto and incorporated herein as Exhibit A. 2. Grant Award. Grant funds in an amount not to exceed one thousand five hundred dollars ($1,500) have been awarded by the Arts Commission to the Grant Recipient for the approved Project. These funds must be matched at least dollar-for-dollar with cash from the Grant Recipient's funds not derived from either Orange County appropriations or the North Carolina Arts Council. 3. Contingency. This grant is contingent on North Carolina Arts Council Grassroots Program funding to the Arts Commission. Final decisions on Grassroots Program funds to the North Carolina Arts Council are subject to approval by the North Carolina General Assembly. 4. Responsibilities of the Grant Recipient 1 Revised May 2012 t A. Implementation. The Project implementation shall take place between January 1, 2014 and December 31, 2014. The Project shall be implemented in the manner described in Exhibit A. Time is of the essence in the implementation of the Project. B. Modifications to project. Grant Recipient shall present major changes to the original proposal, including but not limited to changes in design, implementation schedule and increase in Project budget by more than 10%, in writing to the Arts Commission for approval. Arts Commission shall approve or disapprove Grant Recipient's request for modifications within seven (7) days of receipt thereof. Consent to project modifications shall not be unreasonably withheld. Arts Commission's decisions shall be in writing. In the event the Arts Commission fails to approve or disapprove the requested modifications within the time stated herein,the modifications shall be deemed approved. C. Non—assign ent. The grant cannot be assigned or transferred to a different project or artist without prior written approval of the Arts Commission. D. Records. Grant Recipient must maintain records and other evidence pertaining to costs incurred and revenues acquired during the Project implementation. Grant Recipient shall maintain financial records pertaining to the Project for three (3) years from the date of this Agreement, in case of audit. E. Use of Funds. Grant Recipient shall not use the grant funds to repay outstanding taxes or other debts of any nature. The grants shall be utilized solely for the purpose of implementing the Project and Project related expenses. F. Final Report. Grant Recipient shall submit to the Arts Commission a Final Report on January 16, 2015 . The Final Report shall be in the format required by the North Carolina Arts Council. Failure to submit the Final Report or late submission of the Final Report, for any reason, shall be grounds for denial and/or cancellation of grant funds for the Project and future grant proposals. 5. Responsibilities of the Arts Commission A. Pam. The Arts Commission shall release the grant funds in the amount of $1,500 to the Grant Recipient. 6. Publicity A. Acknowledgement. Credit must be given to the Orange County Arts Commission using the Arts Commission logo and if State funds are received to the North Carolina Grassroots Program in brochures, news releases, programs, and all other printed and online publications. Copies of such printed materials must accompany the evaluation report. When no printed information is used, verbal credit shall be given. In addition, all print and promotional material must include the following credit line(check all that apply): 2 Revised May 2012 t i. x County Funds Only. "This program is supported by the Orange County Arts Commission. " ii. North Carolina Arts Council Funding. "This program is supported by the Orange County Arts Commission with funds from the Grassroots Program of the North Carolina Arts Council, a state agency. " B. The Grant Recipient must enter all events, exhibits and performances open to the public and related to this grant award on the ExploreChape1HillARTS.com website and display ExploreChape1Hi114,RTS.com publicity materials (postcards, brochures, etc.) prominently and consistently at their venue or performance space. In addition, the Grant Recipient must add a link to the ExploreChape1HillARTS.com (ExploreChape1HillARTS.com logo preferred) to their main webpage. The Grant Recipient shall contact the Orange County Arts Commission for replacement ExploreChape1HillARTS.com publicity materials. 7. Indemnity. The Grant Recipient agrees to indemnify and hold harmless the County, the Arts Commission and the North Carolina Arts Council and their officers and employees from any and all claims or actual injury, damage or loss to a person, or real or personal property that results from or is in any way connected to the use of the grant funds. 8. Independent Contractor. The Grant Recipient is an independent contractor and not an employee, agent or other representative of the County and/or the Arts Commission. Nothing contained in this Agreement shall be construed to create the relationship of principal and agent, or employer and employee, between Grant Recipient and Arts Commission or Orange County Government. Grant Recipient understands and agrees that he/she is not authorized to incur any expenses or any liability whatsoever on behalf of the County or the Arts Commission and has no authority, expressed or implied, to obligate or make representations on behalf of the County or the Arts Commission. In addition, Grant Recipient agrees to maintain his or her own worker's compensation insurance for himself/herself and his or her employees for the duration of the art work's installation. 9. Termination A. In the event of any of the circumstances set forth below (hereinafter referred to as "default"),the Arts Commission may immediately terminate this Agreement: (i) Any fraudulent representation in any verification required to obtain payment under this Agreement or other dishonesty on a material matter relating to the performance of services under this Agreement. (ii) Nonperformance, incomplete service or performance, or failure to satisfactorily perform any part of the Project or to comply with any provision of this Agreement, as determined by the Arts Commission in its sole discretion. 3 Revised May 2012 ---i 1 (iii) Failure to adhere to the terms of applicable county, state or federal laws, regulations, or stated public policy. B. In the event of default by the Grant Recipient, the Arts Commission has the discretion to elect to terminate this Agreement, in whole or in part and/or require the Grant Recipient to repay the grant funds within seven (7) from written notice of default or allow the grant recipient the opportunity to cure the default. This clause shall not be interpreted to limit the County's or the Arts Commission's remedies in law or in equity. 10. Miscellaneous. A. Entire Agreement. This Agreement and the attachments contain the final and entire understanding of the parties with respect to the subject matter of this Agreement. All modifications and amendments to this Agreement shall be in writing. This Agreement shall be binding upon the parties, their heirs, successors and assigns. B. Governing Law. This Agreement shall be interpreted in accordance with the laws of the State of North Carolina and the Orange County. Organizational Profile- Mission and History Our mission,since 1991,when The DooR to DooR program of UNC Health Care was founded in association with UNC Hospital Administrators and Physicians,is to bring life and joy and solace to the patients,their families and.the care giving staff by integrating into the hospital environment the gifts of our community's finest artists. Chosen for their artistic excellence and communication skills,and then escorted directly to the patients for visits running from S -20 minutes,well over 2,000 artists have visited well over 100,000 hospital denizens. UNC Hospitals has 804 in-patient beds. DooR to DooR's excellent performing, (cello,piano,guitar, mandolin,violin, accordion, flute, percussion,harp,hammer dulcimer,ukulele,lyre,bass,harmonica and a cappella song) literary, (poet, actress,story teller,rap artist,) and visual artist, (painter,water colorist,photographer, caricaturist, cartoonist,origami and collage maker,) bring their arts into the patients' rooms,as well as Dialysis, Burn Center, Hospital School,Ambulatory Care Center, staff rooms, Oncology and Chemotherapy Clinics,Surgery, Emergency, Pediatric Waiting Areas and lobbies,which become mini-theaters and classrooms. Those visits evoke words like these from patients, staff and our artists: Patients: "It was very uplifting. " Before visit: "Low" After: "Great" Adolescent Psychiatry "I think door to door is a wonderful program. I've been in the burn unit for two weeks now without a single visitor,but having Joy and Teresa come to see me lifted my spirits so so so so much!!!!! Thank you! - Burn Unit, Patient Room She made my day! Awesome!! It's good therapy for me! (My hands don't work very well.) Staff: What a joy to have this group here today! They provided a lighthearted delightful performance for several of our patients,Please return! Thanks, RN A pharmacologist passing by said to me: The music changed my day, lightened my mood. Artists: The door to door program is the most rewarding work I have ever done as a performer. Singing in the cancer patient's room is bringing a direct message of joy,beauty and humanity to someone who desperately needs it,perhaps more than anyone in the world. The results are almost always immediate and life changing. Artists escorted to patient rooms,waiting areas, clinics and units,approximately 10 visits a month. 2. To teach at the UNC child and adolescent, Crisis, Psychosis, Eating Disorders and Geriatric Psychiatric Units: 45 Artists visit usually on Saturday or Sunday. 3. Several Volunteer Artists: Alan Greer with guitar and songs,visits the children 5 days a week. Suzanne Mays, Lyre,visits adults one day a week, Helen Wolfson, Hammered Dulcimer visits adults especially women, one day a month,two conversational clowns,and Roy Schmidt,who has been ill some of this year,but he gives the harmonicas DooR to DoOR purchases for him,to the child patients with some written music and he teaches them to play before he leaves the room. 4. ArtHeels,called "a key to DooR to DooR" during the school year,UNC art majors and others,volunteer to visit mostly child patients with mostly visual arts and clowning and some music. The first year there were 15 volunteers,now in its 4th year there are over 60 volunteers extremely well organized and trained by their student coordinator. They visit patients,mostly pediatric and mostly with visual arts, 6 days a week. 6. DooR to DooR website maintained,photographs added as often as possible taken by director joy Javits and sometimes by others. The designer of the website is Andrea Ferguson, of Ride Again Productions, a supporter! of the program. She donated the design and a year of webhosting. She charged a nominal fee to maintain and update this excellent website: www.doortodoornc.com She had been donating her service again! 7. Maintain the pianos one in the lobby of the NC Children's Hospital and another in the Dining Area for the Rehabilitation Patients. There are now two more pianos, one in the waiting area for Cancer patients and one in the Unit of the Psychosis Adult Psychiatric Patieets. Pianists play at all of them as organized by DooR to DooR and tuning and cleaning are coordinated by DooR to DooR. 8. A Grant received from LiveSTRONG encouraged this Coordinator to work with the patients directly and to make that artistic work interactive. Becaue of that excellent training,many DooR to DooR artists work more interactively with the patients. For example Gabriel Pelli brought his fiddle to play and also brought a small guitar which he tuned with an open string and that allowed several patients who said,"I'm not at all musical" to have a go at playing the guitar. It also allowed a Burn patient/musician to play with Gabriel for several other patients. Natalie Gilbert teaches patients in Psychosis and Geriatrics to play the piano and play with each other. Billy Stewart gave mini-guitar lessons to patients. Proposed Uses of Grant Funds DooR to DooR has been embraced especially by the UNC Psychiatric Units including: Geriatric, Crisis, Psychosis, Eating Disorders, Adolescent and Child. I am requesting $800 to give an honorarium to ten artists to perform in the psychiatric units on the weekends, and $700 to invite dancers and actors to participate with patients, performing and interactively. Funding for Artists,parking,supplies: $10,000 (thrilled - allows 90 visits by artists over the year. We make it to each location every two months,except for Psychiatry with the focus this year. Grants for Artists: Orange County Arts Commission for 15 artists,new focus on dance and adding to this exciting focus on psychiatry. Attachment of comments by the young patients from a visit just this past Saturday,November 9, 2013 $1,500 Pending Grant for Artists: Strowd Roses, Inc. Trying for a repeat grant from this wonderful local foundation. Again the request will be for artists visits with an interactive focus,visual artists to make art with patients,musical artists to bring lyrics or sing requests by patients, literary artists to bring a poem or story to give the patient after the reading,or to write together. $4,000 Pending My goal is to have an at least four artists visiting each week of the year. Budget would need to be: $20,000 plus parking. Also,wish to increase the funds paid to the Coordinator because I don't know that anyone would accept what I do. Looking for$21,000 For a TOTAL: $42,000 Am about to work with a colleague on fund raising from individuals. Right now, my sister and brother and friends give most of the donations I receive. Sometimes new members of the UNC community contribute too! With the generous contribution of departments such as Volunteer Services at UNC Hospitals, Door to Door services were recently expanded upon in Psychiatry Services.As Director of Psychiatry Services, I could not personally be more pleased with the results. First there is the anecdotal evidence of smiling faces of patients and employees alike from the geriatric to the children's units when the artists and performers visit. However,scientific research has shown that an involvement in the healing arts leads to decreased symptomatology for many patients with a diagnosis of a mental illness and can lead to shorter hospital stays and better outcomes after discharge. Having the involvement of this program in our inpatient program should not be purely an exception but more of an essential part of their treatment program.This is one of the components of UNC Hospitals that sets us apart from other hospitals in regard to the holistic approach and evidence based treatment in the care we provide.Thank you to our generous supporters who have helped these services expand in Psychiatry;thank you to the endless enthusiasm and passion of founder and coordinator Joy Javits and especially to artists and performers that help instill hope'in our patients when they are at times at the worst moments in their lives.
