BACKGROUND: The following appointments are for Board consideration: • Appointment to a second full term for Mr. Daniel Hatley. If appointed, Mr. Hatley will be serving a second full term expiring 10/31/2015. • Appointment to a first full term for Dr. Anthony John Vogt. Dr. Vogt completed a one- year training term expiring 10/18/2013. If appointed, Dr. Vogt will be serving a first full term expiring 10/31/2015. • Appointment to a third full term for Mr. Richard Gross. Daniel Hatley or a second full term. Mr. Hatley has provided strop leadership to the committee in his Y Y l3 is l' role as chair and he is a major asset to the committee's success. He is a strong advocate for the elderly and disabled individuals living in long term care facilities and serves as a role model for the members of this committee. Mr. Hatley has attended 6 out of 7 business meetings during the past year, completed all quarterly facility visits that were assigned to his subgroup, and participated in several leadership sessions offered by the Ombudsman program. Lastly Mr. Hatley has directly participated in several advocacy projects that the committee has sponsored and encourages other members to do the same. I Dr. Anthony John Vogt for a first full term. During this past year Dr. Vogt completed the CAC new member orientation, attended 100% of the committee's business meetings, participated in all of his assigned quarterly facility visitations and volunteered to participated on several committee projects. His passion for the elderly is evident in his committee work. i Richard Gross for a third full term. During this past year Mr. Gross attended 5 out of 7 business meetings and participated in all of his assigned quarterly facility visitations.
