COUNTY OF ORANGE
RENEWAL AGREEMENT BETWEEN
ORANGE COUNTY HEALTH DEPARTMENT
AND
JOINT ORANGE-CHATHAM COMMUNITY ACTION AGENCY
THIS AGREEMENT,made and entered into this the 1 st day of January, 1997 by and between the Orange
County Health Department,hereinafter referred to as the"OCHD,"and the Joint Orange-Chatham Community
Action Agency,hereinafter referred to as the"JOCCA."
WITNESSETH:
WHEREAS,an agreement was made and entered into as of the 1st day of October, 1995 by and between the
OCHD and JOCCA to implement the goals of the W.K. Kellogg Foundation Community-Based Public Health
Project in Orange County.
NOW, THEREFORE, in consideration of the premises and of the following mutual promises, covenants,and
conditions, OCHD and JOCCA agree as follows:
1. The Agreement will be continued ot'i a month to month basis beginning January 1, 1997 for a period not
to exceed three months.
2. Payment shall not exceed$6,878.09.
3. All equipment acquired with funds allocated by this agreement and preceding agreements shall be
returned to OCHD upon final payment.
3. All other terms and conditions shall remain the same.
IN WITNESS WHEREOF,the parties have hereunto signed this Agreement in their official capacities on
the day and year listed below.
