Purpose:
To formalize in a written agreement, an existing arrangement between the Health Department and
the DMA for the Health Department to provide Health Check Outreach Services in return for
$2,922 dollars per month for each approved coordinator position.
Background:
The Health Check Program was established to increase the number of Medicaid eligible children
who were receiving regular health checkups. The Division of Medical Assistance recognized that
the cost to provide both the outreach and the Health Check service was less than the cost to
provide medical care for children when their health problems were not recognized early. Studies
on the initial, pilot projects have proven to achieve successful outcomes resulting in the DMA
offering stable funding for all health departments to adopt this program.
Orange County was one of the pilot projects. All of the newer projects have signed the attached
agreement. Therefore, Orange County and the other pilot projects are also being asked to
endorse the agreement.
Recommendation: For the Board of Commissioners to approve the Health
Check Outreach Project implementation agreement and authorize the
Health Director to sign the document in accordance with the policy
previously approved by the Commissioners.

HEALTH CHECK OUTREACH PROJECT
IMPLEMENTATION AGREEMENT
This agreement between the North Carolina Department of Human Resources,
Division of Medical Assistance(DMA),and the Ors0ge Cotnt9 Hmdth Dep-t—umt
(agency) is to establish and maintain a Health Check Outreach Project in
Die County.
The Division of Medical Assistance agrees to:
1. Reimburse$2,922 per month for each approved coordinator position.
Reimbursement for Health Check Coordinator position(s)is initiated at the
end of the first month of employment.and provided at the end of each
month thereafter for each filled position:
2. Provide ongoing Health Check Program guidance and consultation:
3. Provide Health Check Introductory Training for Health Check
Coordinator(s)and Supervisor(s):and
4. Implement and maintain the Automated Information and Notification
System(AINS)in the county subsequent to required training.

1. Assure that each Health Check Coordinator and Supervisor will attend the
Health Check Introductory Training within the time frames agreed upon
with DMA;
2. Implement and operate the Health Check Project in accordance with Health
Check Program policies and procedures.

3. Serve the entire county by providing Health Check coordination services to
families of Medicaid-eligible children from birth through age 20;
4. Assure that Health Check Coordinator(s)devote 100%of their time to Health
Check Program activities. Coordinators are required to dedicate 50%of their
time to direct client contact. Client related activities,including direct client
contact,must account for 75%of Coordinator time;
5. Prepare appropriate job description(s) for Health Check Coordinator(s)
according to DMA requirements prior to initiation of reimbursement;
6. Purchase appropriate hardware(PC)and software for utilization of AINS data.
These purchases would be supported by the county Health Check Coordinator
reimbursement which includes funding for administrative overhead.
This agreement is effective for two years, beginning April 1, 1997 and ending
March 31, 1999,contingent on a successful annual review at the end of the first year.
The agreement is renewable, thereafter,contingent upon successful annual reviews
and compliance with program policies. The agreement may be terminated by either
party upon(60)days written notice.
