Time Warner Cable Business Class Optical Access Addendum to Service Agreement
This Business Class Optical Access Addendum to Service Agreement[Contract#01523357 is made by and between Time Warner Cable,Raleigh

Services: Subject to the terms and conditions of Service Agreement[Contract#],TWC shall provide Customer with a dedicated fiber optic
network(as detailed in Exhibit A) Customer may purchase any additional capacity separately.
Total Costs Added to Master Service Agreement: $435.50
Service Change Fee:$300.00
The fees set forth above do not include applicable taxes and other similar charges (as described more fully in the Time Warner Cable
Business Class Optical Access Service Agreement Terms and Conditions)which may be part of the fee charged by TWC hereunder and
which shall be the responsibility of the Customer as set forth in this Agreement. As of January 1,2010 FUSF and FCC fees are estimated
at 15.56%. This amount is subject to change without notice.
Term of Service: is co-terminus with Contract#01523357 from September 27,2012 through September 27,2014
THIS BUSINESS CLASS OPTICAL ACCESS SERVICE AGREEMENT SHALL AT ALL TIMES BE SUBJECT TO THE TERMS AND
CONDITIONS,LOCATED AT http://raleigh.twcbc.com/policy/legal.cfm(AS SUCH AGREEMENT MAY BE MODIFIED FROM TIME TO TIME AS
SPECIFIED THEREIN),WHICH TERMS AND CONDITIONS ARE HEREBY INCORPORATED HEREIN BY REFERENCE AND MADE A PART
OF THIS AGREEMENT FOR ALL PURPOSES. BY EXECUTING THIS AGREEMENT, CUSTOMER ACKNOWLEDGES THAT CUSTOMER
HAS READ AND AGREES TO BE BOUND BY SUCH TERMS AND CONDITIONS.
