THIS AGREEMENT, entered into this 19th day of July, 2012, by and between Orange County
("County"), by and through the Orange County Arts Commission ("Arts Commission") and Door
to Door of UNC Health Care("Grant Recipient").
WHEREAS, the Arts Commission is a countywide arts agency that ensures the growth of
local arts experiences by providing funding and support for diverse high quality programs and
projects.
WHEREAS, the Arts Commission administers the Spring and Fall Arts Grants Program
to provide economic opportunities to Orange County artists and non-profit organizations
coordinating arts projects benefiting the citizens of Orange County.
WHEREAS, the Spring Arts Grants Program is supported by Arts Commission with
funds from the Grassroots Program of the North Carolina Arts Council, an agency of the
Department of Cultural Resources and the National Endowment for the Arts.
WHEREAS, the Grant Recipient was selected by the Arts Commission to receive the
Spring Arts Grant through the procedures duly adopted by the Arts Commission; and
WHEREAS, both parties wish to promote and maintain the integrity and clarity of the
Grant Recipient's ideas and statements and delineate the Grant Recipient's obligations to the
County;
NOW, THEREFORE, the County and the Grant Recipient, for the consideration and
under the conditions hereinafter set forth,agree as follows:
1. Approved Grant Project. Grant funds are provided under this Agreement for the project
("Project") described in the Grant Recipient's Grant Application, attached hereto and
incorporated herein as Exhibit A.
2. Grant Award. Grant funds in an amount not to exceed one thousand five hundred dollars
($1,500) have been awarded by the Arts Commission to the Grant Recipient for the
approved Project. These funds must be matched at least dollar-for-dollar with cash from
the Grant Recipient's funds not derived from either Orange County appropriations or the
North Carolina Arts Council.
3. Contingency. This grant is contingent on North Carolina Arts Council Grassroots
Program funding to the Arts Commission. Final decisions on Grassroots Program funds
to the North Carolina Arts Council are subject to approval by the North Carolina General
Assembly.
4. Responsibilities of the Grant Recipient

A. IMplementation. The Project implementation shall take place between July 1,
2012 and June 30, 2013. The Project shall be implemented in the manner
described in Exhibit A. Time is of the essence in the implementation of the
B. Modifications to pM�ect. Grant Recipient shall present major changes to the
original proposal, including but not limited to changes in design, implementation
schedule and increase in Project budget by more than 10%, in writing to the Arts
Commission for approval. Arts Commission shall approve or disapprove Grant
Recipient's request for modifications within seven (7) days of receipt thereof
Consent to project modifications shall not be unreasonably withheld. Arts
Commission's decisions shall be in writing. In the event the Arts Commission
fails to approve or disapprove the requested modifications within the time stated
herein, the modifications shall be deemed approved.
C. Non—assignme . The grant cannot be assigned or transferred to a different project
or artist without prior written approval of the Arts Commission.
D. Records. Grant Recipient must maintain records and other evidence pertaining
to costs incurred and revenues acquired during the Project implementation. Grant
Recipient shall maintain financial records pertaining to the Project for three (3)
years from the date of this Agreement, in case of audit.
E. Use of Funds. Grant Recipient shall not use the grant funds to repay outstanding
taxes or other debts of any nature. The grants shall be utilized solely for the
purpose of implementing the Project and Project related expenses.
F. Final Rep.2rt. Grant Recipient shall submit to the Arts Commission a Final Report
on or before Friday, July 5, 2013 . The Final Report shall be in the format
required by the North Carolina Arts Council. Failure to submit the Final Report
or late submission of the Final Report, for any reason, shall be grounds for denial
and/or cancellation of grant funds for the Project and future grant proposals.
5. Responsibilities of the Arts Commission
A. PUme . The Arts Commission shall release the grant ftmds in the amount of
$1,500 to the Grant Recipient.
6. Publicity
A. Acknowledaeme Credit must be given to the Orange County Arts
Commission using the Arts Commission logo and if State funds are received
to the North Carolina Grassroots Program in brochures, news releases,
programs, and all other printed and online publications. Copies of such printed
materials must accompany the evaluation report. When no printed information is
used, verbal credit shall be given. In addition, all print and promotional material
must include the following credit line (check all that apply):

i. County Funds Only. "This program is supported by the Orange
County Arts Commission. "
ii. x North Carolina Arts Council Funding. "This program is supported by the
Orange County Arts Commission with funds from the Grassroots Program of
the North Carolina Arts Council, a state agency. "
B. The Grant Recipient must enter all events, exhibits and performances open to the
public and related to this grant award on the ExploreChapelHillARTS.com
website and display ExploreChapelHillARTS.com publicity materials
(postcards, brochures, etc.) prominently and consistently at their venue or
performance space. In addition, the Grant Recipient must add a link to the
ExploreChapelHillARTS.com (ExploreChapelHillARTS.com logo preferred)
to their main webpage. The Grant Recipient shall contact the Orange County Arts
Commission for replacement ExploreChapelHiIIARTS.com publicity materials.
7. Indemnity. The Grant Recipient agrees to indemnify and hold harmless the County, the
Arts Commission and the North Carolina Arts Council and their officers and employees
from any and all claims or actual injury, damage or loss to a person, or real or personal
property that results from or is in any way connected to the use of the grant funds.
8. Independent Contractor. The Grant Recipient is an independent contractor and not an
employee, agent or other representative of the County and/or the Arts Commission.
Nothing contained in this Agreement shall be construed to create the relationship of
principal and agent, or employer and employee, between Grant Recipient and Arts
Commission or Orange County Government. Grant Recipient understands and agrees
that he/she is not authorized to incur any expenses or any liability whatsoever on behalf
of the County or the Arts Commission and has no authority, expressed or implied, to
obligate or make representations on behalf of the County or the Arts Commission. In
addition, Grant Recipient agrees to maintain his or her own worker's compensation
insurance for himself/herself and his or her employees for the duration of the art work's
installation.
9. Termination
A. In the event of any of the circumstances set forth below(hereinafter referred to as
"default"),the Arts Commission may immediately terminate this Agreement:
(i) Any fraudulent representation in any verification required to obtain
payment under this Agreement or other dishonesty on a material matter
relating to the performance of services under this Agreement.
(ii) Nonperformance, incomplete service or performance, or failure to
satisfactorily perform any part of the Project or to comply with any
provision of this Agreement, as determined by the Arts Commission in its
sole discretion.

(iii) Failure to adhere to the terms of applicable county, state or federal laws,
regulations, or stated public policy.
B. In the event of default by the Grant Recipient, the Arts Commission has the
discretion to elect to terminate this Agreement, in whole or in part and/or require
the Grant Recipient to repay the grant funds within seven (7) from written notice
of default or allow the grant recipient the opportunity to cure the default. This
clause shall not be interpreted to limit the County's or the Arts Commission's
remedies in law or in equity.
10. Miscellaneous.
A. Entire Agreement. This Agreement and the attachments contain the final and
entire understanding of the parties with respect to the subject matter of this
Agreement. All modifications and amendments to this Agreement shall be in
writing. This Agreement shall be binding upon the parties, their heirs, successors
and assigns.
B. Governing Law. This Agreement shall be interpreted in accordance with the laws
of the State of North Carolina and the Orange County.
IN WITNESS WHEREOF, the Orange County and the Grant Recipient have signed this Grant
Agreement, effective on the last date this Agreement is signed by both parties as indicated by the
dates set forth under signatures below.

DooR program, I request$1,500 to offer an honorarium to fifteen of them to bring their poignancy: Ericka
Lee Wilkins, Classical Songs, Harvey Dalton Arnold, Guitar and Rock Ballads Kim Arrington Writer with
Psychiatric child and adult patients; their humor: Charlotte Runde, Caricature art that gives back a cancer
patient's hair, Becky Lofland with Androgeena Puppet, who is 7 for the 21S time Joshua Lozoff, Actor and
Magician, and their surprise: Steven Fishmnan, sings songs from the 30's &40's Jenifer Crowell a
clown who draws beautiful paintings on patient faces or arms, James Beckwith who sits on a pillow on the
floor and plays the sitar and a tango dancing couple, Hispanic singer, harmonica harp and bass players
and poet; to the patients and their caregiving families, friends and staff at UNC Health Care.

NARRATIVE
For 2012-2013 Grant
Organizational Profile-Mission and History
Our mission since 1991,when The DooR to DooR program of UNC Health Care was founded in association with UNC Hospital
Administrators and Physicians, is to bring life and joy and solace to the patients,their families and the care giving staff by integrating
into the hospital environment the gifts of our finest artists. Chosen for their artistic excellence and communication skills, and then
escorted directly to the patients for visits running from 3-30 minutes,over 2,000 artists have visited well over 100,000 hospital
denizens. UNC Hospitals has 688 in patient beds and many thousands of visitors,medical and assisting staff.
Door to DooR's excellent performing,(cello,piano,guitar,mandolin,violin,sitar,accordion, flute,percussion,harp,hammer
dulcimer,ukulele, lyre,bass,harmonica and a cappella song)literary,(poet,actor,comedian,story teller,rap artist,)and visual artist,
(painter,water colorist,photographer,caricaturist,cartoonist,origami and collage maker,)bring their arts into the patients' rooms,as
well as Psychiatric Units,Burn Center,Ambulatory Care Center,staff rooms,Oncology and Chemotherapy Clinics,Surgery,
Emergency,Pediatric Waiting Areas and lobbies,which become mini-theaters and classrooms. Those visits evoke words like these
from patients,staff and our artists:

Artists:
The door to door program is the most rewarding work I have ever done as a performer. Singing in the cancer patient's room is
bringing a direct message of joy,beauty and humanity to someone who desperately needs it,perhaps more than anyone in the world.
The results are almost always immediate and life changing. Artist: Jonathan Byrd,Guitar and Original Songs
I've been playing music for a long time,but playing for DooR to DooR has made me a musician.

Current Programs and Services-a brief List
1. Artists escorted to patient rooms,waiting areas,clinics and units,approximately 10 visits a month,120 visits a year.
2. 10 Artists visit on a rotating basis to work with the child,adolescent,eating disorders,crisis,psychosis and geriatric units
almost all on the weekends.
3. Drumming Circle once a month with a Master Drummer. Two-one hour drumming sessions at two of the Psychiatric in-
patient wards.
4. Several Volunteer Artists: Alan Grier,guitar and songs,visits the children 5 days a week. Suzanne Mays,Lyre,visits adults
in Bone Marrow and Intensive Care,one day a week,Helen Wolfson,Hammered Dulcimer visits adults especially women,one
day a month. Roy Schmidt,gives an harmonica and book of music to the pediatric patients. He teaches children to play the
harmonica before he leaves.
5. ArtHeels,called"a key to DooR to DooR"during the school year,UNC art majors and others,volunteer to visit mostly child
patients with mostly visual arts and clowning and some music. The first year there were 15 volunteers,by second year second
semester there were 50 volunteers extremely well organized by Senior,Katy Heubel. They visit patients 6 days a week. The
new head of ArtHeels takes over in the Autumn. They are a Club at UNCH and even more interest is growing in the student
population.
6. DooR to DooR website maintained,photographs added as often as possible taken by director Joy Javits and sometimes by
others. The designer of the website is Andrea Ferguson,of Ride Again Productions,a supporter of the program. She donated
the design and a year of webhosting. She charges a nominal fee to maintain and update this excellent website:

7. Maintain the pianos one in the lobby of the NC Children's Hospital,one in the Crisis Psychiatric Unit and one more in the
Dining Area for the Rehabilitation Patients. Pianists play there,tuning and cleaning are coordinated by DooR to DooR.
artists work a bit more interactively with the patients. For example,Gabriel Pelli brought his fiddle to play and also brought a
small guitar which he tuned with an open string and that allowed several patients who said,"I'm not at all musical"to have a

8. With funds from the LiveSTRONG Foundation,painting workshops were offered to the Oncology Patients,by Jane Filer and
iPad art to the Pediatric Oncology patients,by Bridget Pemberton-Smith. Collage and poetry writing were offered by Joy Javits
to the adult Oncology patients in the hospital chemotherapy and patient rooms. The Grant received from LiveSTRONG
encourages this Coordinator to work with the patients directly and to make that artistic work interactive. The DooR to DooR
go at playing the guitar. It also allowed a Burn patient/musician to play with Gabriel for several other patients. More art is
being made interactively. The guitar,keyboard are made available to patients to borrow on a regular basis. A first year
medical student offers piano lessons to patients. The new harmonica program is very interactive.
Proposed Uses of Grant Fund

DooR to DooR is in its 21St year!
The fifteen artists will perform at UNC Hospitals during the 2012-2013 year. Diali Cissokho and Pline Mounzeo will give
drumming sessions to the psychiatric patients. The 10 Djembe drums we use were purchased by the UNC Volunteer
Association,a huge support to the arts in the hospital,some years ago for use by the Physical Therapist who started this drum
circle,Frake Hansel,Physical Therapist. Occupational Therapist,Julie Pace,often accompanies the drummers and gives her OT
time for the drumming. Louise Omoto Kessel will offer collage,puppet making and origami making to Pediatric Patients.
Ericka Lee Wilkins will sing Classical Songs to women on the 3rd(women on bed rest)and 6th(Women with Cancer and other
ailments. Harvey Dalton Arnold,who was with the Nationally acclaimed"Outlaws"band,will play his Guitar and Rock Ballads,
Harvey has been a patient. Kim Arrington,a Poet will work with Psychiatric child and adult patients;she hopes to put together
of book of their writings. Charlotte Runde will draw,in 5 minutes flat,a Caricature portrait that she gives to the patient. They
smile when they see how she put back as it was,a cancer patient's hair,or removes the tubes. Becky Lofland with Androgeena
Puppet,who is 7 for the 21St time,sports a watch on her ankle,"so the cats can see when it's snack time."Richard Tazwell,
sings and plays the keyboard. He brings strange and interesting instruments like the Jamaican thumb piano,portable roll-up
piano and backpack guitar,cymbals and cowbell for patients to play,which often make them laugh. Steven Fishman,sings
songs from the 30's&40's,Jenifer Crowell,a silent clown/artist draws beautiful paintings on patient faces or arms,James
Beckwith sits on a pillow on the floor and plays the Sitar and an Hispanic singer,Teresa Fernandez,acoustic Bass player,Ben
Palmer and a Writer,Megan Cutter,she works with her husband who is disabled,and very funny.
Several of the artists will be performing for the Psychiatric patients. They are Children and Adolescents,Eating Disorders,
Crisis,Psychosis and Geriatric patients,all locked in. Others of the artists will travel to the various areas of where patients are
in separate rooms. Nurses will recommend,and I will ask permission of each patient.
The patients tend to be from North Carolina but some are from neighboring states. Race,ethnicity and gender are divided for
the most part between males and females,Caucasian and African American patients. More occasionally there is a patient or
staff member who is Asian,Native American or Indian.
