BACKGROUND: The State Department of Human Resources established the Home and Community Care Block Grant in July 1, 1992. The grant did not create new funding streams, but combined funds appropriated to the Department on Aging, Department of Social Services, and Joint Orange Chatham Community Action Agency, (JOCCA) to administer programs for low-income minority elderly and rural elderly. The Block Grant requires the Board of Commissioners to approve the recommended funding plan. The recommended funding for FY 2003-2004 is $429,772. This amount represents an increase of $3,743 over the 2002-2003 Block Grant amount of $426,029. The Home and Community Care Technical Advisory Committee composed of representative from DSS, JOCCA, the DOA, and the County Manager's Office met and reviewed the plan on April 8, 2003, and supported the continuation of current outreach and services delivery. The Advisory Board on Aging also reviewed the committee's plan on May 9, 2003 and recommended approval. The recommended Funding Plan is attached. FINANCIAL IMPACT: The Home and Community Care Block Grant for FY 2003 - 2004 provides for a total of $429,772 to support aging services in Orange County. RECOMMENDATION(S): The Manager recommends that the Board approve the 2003-2004 Home and Community Care Block Grant Funding Plan and authorize the Chair to sign grant documents. Home and Community Care Block Grant Funding Comparison for FY 2002-2003 and FY 2003-2004 Programs Block Grant Proposed Block Funding Grant Funding FY-2002-2003 FY 2003-2004 JOCCA Congregate Nutrition JOCCA has six Congregate Meal Sites located in $103,716 $103,716 Orange County. A site manager is located at each site and is responsible for the daily operation of nutrition programs and meals. There are also three coordinators of services responsible for the coordination, outreach, referral and provision of services to low-income families and individuals. These services include, but are not limited to weatherization, housing rehabilitation assistance, financial emergency assistance, Older Workers Pro ram, and information and referral. DSS In-Home Aid Services $ 33,928 $ 34,599 Home and Community Block Grant enables low- income, minority individuals to defer or prevent institutionalization by providing quality services to meet basic needs. These services include the performance of essential home management and personal care tasks rendered under guidance and su envision. The community service provider shall specify how the service needs oflow-income minority elderly and rural elderly will be met through the services identified on the Provider Services Summary (DOA-732). This narrative shall address outreach and service delivery methodologies that will ensure that this target population is adequately served and conform with specific objectives established by the Area Agency on Aging, for providing services to low income minority individuals. Additional pages may be used as necessary. 8 DOA-734 (revised 1/99) July 1, 2003 through June 30, 2004 Home and Community Care Block Grant for Older Adults Community Service Provider Standard Assurances Orange County DSS agrees to provide services through the Home and (Name of Provider) Community Care Block Grant, as specified on the Provider Services Summary (DOA-732) in accordance with the following: 1. Services shall be provided in accordance with requirements set forth in: a) The County Funding Plan; b) The Division of Aging Home and Community Care Block Grant Procedures Manual for Community Service Providers; and c) The Division of Aging Services Standards Manual, Volumes I through N. Community service providers shall monitor any contracts with providers of Block Grant services and take appropriate measures to ensure that services are provided in accordance with the aforementioned documents. 2. Priority shall be given to providing services to those older persons with the greatest economic or social needs, with particulaz attention to low-income minority individuals. The service needs oflow-income minority elderly will be addressed in the manner specified on the Methodolo~y to Address Service Needs ofLow-Income 1Vlinorit E~ lderly format, (DOA-733). 3. The following service authorization activities will be carried out in conjunction with all services provided through the Block Grant: a) Eligibility determination; b) Client intake/registration; c) Client assessment/reassessments and quarterly visits, as appropriate; d) Determining the amount of services to be received by the client; and e) Reviewing cost sharing/voluntary contributions policies with eligible clients. 4. All licenses, permits, bonds, and insurance necessary for carrying out Block Grant Services will be maintained by the community service provider and any contracted 9 DOA-734 (revised 1/99) providers. 5. As specified in 45 CFR 92.36(b)(11), community service providers shall have procedures for settling all contractual and administrative issues arising out of procurement of services through the Block Grant. Community service providers shall have procedures governing the evaluation of bids for services and procedures through which bidders and contracted providers may appeal or dispute a decision made by the community service provider. 6. Applicant/Client appeals shall be addressed as specified in Section 7 of the Division of Aging Home and Community Care Block Grant Manual for Community Service Providers, dated February 17, 1997. 7. Community service providers are responsible for providing or arranging for the provision of required local match, as specified on the Provider Services Summary (DOA-732). Local match shall be expended simultaneously with Block Grant funding. 8. Providers expending $300,000 or more in federal financial assistance through the Home and Community Care Block Grant, or in combination with other federal funding shall receive an annual independent audit which meets the requirements of the Division of Aging Program Audit Guide for Aging Services, applicable North Carolina General Statutes and Local Government Commission requirements, and OMB Circulaz A-133. For-profit community service providers shall have an annual compliance audit which meets the requirements of A-133. The audit shall be performed within nine (9) -- months of the close of the provider's fiscal year. Upon completion of the audit, non- profit and for-profit providers shall provide a copy of the audit report and any opinion letter simultaneously to the County and the Area Agency. Federal funds will not reimburse the cost of a single audit if the total of all federal funds expended by the provider is less than $300,000 9. Compliance with Equal Employment Opportunity and Americans With Disabilities t Act requirements, as specified in pazagraph fourteen (14) of the Agreement for the Provision.of County Aging-Based Services (DOA-735) shall be maintained. 10 DOA-734 (revised 1/99) 10. Providers In-Home Aide, Home Health, Housing and Home Improvement, and Adult Day Caze or Adult Day Health Care shall sign and return the attached assurance to the area agency on aging indicating that recipients of these services have been informed of their client rights, as required in Section 314 of the 1992 Amendments to the Older Americans Act. (Authorized Signature) (Date) 11 Standard Assurance to Comply with Older Americans Act Requirements Regarding Client Rights for Agencies Providing In-Home Services through the Home and Community Care Block Grant for Older Adults As a provider of one or more of the services listed below, our agency agrees to notify all Home and Community Care Block Grant clients receiving any of the below listed services provided by this agency of the their rights as a service recipient. Services included in this assurance include: -In-Home Aide -Home Care (home health) -Housing and Home Improvement -Adult Day Care or Adult Day Health Care Notification will include, at a minimum, an oral review of the information outlined below as well as providing each service recipient with a copy of the information in written form. In addition, _ providers of in-home services will establish a procedure to document that client rights information has been discussed with in-home services clients (e.g. copy of signed Client Bill of Rights statement). Client Rights information to be communicated to service recipients will include, at a minimum, the right to: -be fully informed, in advance, about each in-home service to be provided and any change in service(s) that may affect the well-being of the participate; -participate in planning and changing any in-home service provided unless the client is adjudicated incompetent; -voice a grievance with respect to service that is or fails to be provided, without discrimination or reprisal as a result of voicing a grievance; -confidentiality of records relating to the individual; -have property treated with respect; and -be fully informed both orally and in 'writing, in advance of receiving an in-home service, of the individual's rights and obligations. Client Rights will be distributed to, and discussed with, each 'new client receiving one or more of the above listed services prior to the on-set of service. For all existing clients, the above information will be provided no later than the next regularly scheduled service reassessment. The Orange County Department on Aging, while providing services to any County resident sixty years and older, makes special efforts to reach those individuals who are low-income minority and rural elderly. The Department accomplishes this through the following methods: • Coordinate services through the four JOCCA-operated senior meal sites, providing facility space and access services in these locations. Fixed transportation routes are operated for those who reside outside the municipal bus service area. The Information Specialist makes monthly visits to each of the meal sites to remind participants of the services available and assist individuals in accessing resources to meet their needs. • Provide low and no-cost programs and activities at each of the three senior centers, with scholarships available for those activities that have a fee attached. These offerings are advertised in the free departmental bi-monthly publication, the Senior Times. The Senior Times is distributed at the centers and inserted in the Satuday edition of the Chapel Hill Herald with a distribution of approximately 25,000 households in Orange County. • Work closely with the Department of Social Services to assure that in-home and institutional respite care services are targeted to those low-income individuals who could benefit. • Make presentations to church groups in rural Orange County to inform residents of services for seniors and to promote volunteerism through the Friend-to-Friend Program. 14 • Use UNC-CH students to provide outreach to targeted neighborhoods, which affords students the opportunity to gain valuable experience in community outreach while maximizing outreach funding.. • Continue the extensive publicity campaign to make Orange County senior citizens aware that all of the resources available to them regardless of their geographic location. This includes distributing Eldercare Community Resource Guides to rural civic and church groups. The community service provider shall specify how the service needs of low-income minority elderly and rural elderly will be met through the services identified on the Provider Services Summary (DOA-732). This narrative shall address outreach and service methodologies that will ensure that this target population is adequately served and conform with specific objectives established by the Area Agency on Aging, for providing services to low-income minority individuals. Additional pages may be used as necessary. Joint Orange-Chatham Community Action, Inc. (JOCCA) was established in 1966 and built on a foundation of citizen participation with constituents from all wallcs of life: poor and rich; black and white; public and non-public. The agency is dedicated to serving the needs of all very low and low- income residents of Orange and Chatham counties. The Agency's mission is "to help people help themselves" and attain a status of financial independence through self-help initiatives. JOCCA is a vital part of lives of thousands of very low and low-income residents and plays a salient part in their everyday existence.. To meet the needs of clients, JOCCA relies on the. coordinated efforts and partnerships with other human service agencies, the private sector, local governments and private foundations 21 The Nutrition Program Director and Nutrition Site Managers aze responsible for the day-to day supervision of the Congregate Nutrition Program in Orange County. In cooperative agreements with the Orange County Department on Aging, LTNC School of Nursing, local Health and Recreation Departments, Cooperative Extension, North State Legal Services, Durham Technical Community College, and the N.C. Division of Aging, JOCCA is able to provide but not limited to the following program services to its participants. Creative Arts and Crafts including Games Nutrition/Health Education Classes Assessments of Vital Signs Preliminary Health Screening-e.g. Blood Pressure and Diabetic Checks Legal Services Adult Basic Services (ABE) General Equivalency Diploma (GED) Annual May Day Programs Health Fairs Crime Prevention Workshops Rape Prevention Workshops Transportation to Congregate Nutrition Sites Shopping and Recreational Trips The Congregate Nutrition Program is advertised in the local newspapers. JOCCA's brochures are also disseminated through out the service delivery area. Announcements are also made at all interagency meetings about the luncheon program. Referrals for program participation are received from cognate human service agencies, community residents, local community churches, civic organizations and current program participants. Priority consideration for service (s) will be given to older individuals who exemplify the greatest socioeconomic need; special attention will be given to low-income minority and rural elderly individuals. The following priority system will be utilized. (1) Highest priority will be given to the abused, neglected and /or exploited and for whom the service is needed as identified by DSS; (2) Individuals at risk of abuse, neglect and/or exploitation; (3) Individuals who do not have a care provider or another responsible party to assist with care; (4) Individuals who experience impairment in performance of daily living. 22 JOCCA has [5] Congregate Meal Sites located in Orange County. In 2002 we served 31,263 meals to 478 unduplicated participants. A site manager is located at each site and is responsible for the daily operation of nutrition program. There is also a CSBG Case Manager and an Intake Specialist who are mainly responsible for the coordination, outreach, referral and provision of services to low-income families and individuals. These services include but aze not limited to: Weatherization, Emergency Assistance, Title V Older Worker's Program, and Information and Referral In conclusion, JOCCA relies heavily on interagency coordination, cooperation and resource mobilization and/or fund leveraging. During the past yeaz, there has been a significant increase in the demand for services, especially from the "working poor". In order to continue to meet this demand, JOCCA must intensify its resource mobilization efforts through the solicitation of funds from federal, state, local and private sources; intensify its utilization of volunteers; and enhance its fundraising efforts. 23 DOA-734 ~ (revised 1/99) July 1, 2003 through June 30, 2004 Home and Community Care Block Grant for Older Adults Community Service Provider Standard Assurances int Orange-Chatham Communitv Action. Inc agrees to provide services through the Home and (Name of Provider) Community Care Block Grant, as specified on the Provider Services Summary (DOA-732) in accordance with the following: 1. Services shall be provided in accordance with requirements set forth in: a) The County Funding Plan; b) The Division of Aging Home and Community Care Block Grant Procedures Manual for Community Service Providers; and c) The Division of Aging Services Standazds Ivianual, Volumes I through IV. Community service providers shall monitor any contracts with providers of Block Grant services and take appropriate measures to ensure that services aze provided in accordance with the aforementioned documents. 2. Priority shall be given to providing services to those older persons with the greatest economic or social needs, with particulaz attention to low-income minority individuals. The service needs oflow-income minority elderly will be addressed in the manner specified on the Methodolo~y to Address Service Needs ofLow-Income Minori Elderly format, (DOA-733). 3. The following service authorization activities will be carried out in conjunction with all services provided through the Block Grant: a} Eligibility determination; b) Client intake/registration; c) Client assessmentlreassessments and quarterly visits, as appropriate; d) Determining the amount of services to be received by the client; and e)~ Reviewing cost sharing/voluntary contributions policies with eligible clients. 4. All licenses, permits, bonds, and insurance necessary for carrying out Block Grant Services will be maintained by the convnunity service provider and any contracted 24 DOA-734 ~ (revised 1/99) providers. 5. As specified in 45 CFR 92.36(6)(11), community service providers shall have procedures for settling all contractual and administrative issues arising out of procurement of services through the Block Grant. Community service providers shall have procedures governing the evaluation of bids for services and procedures through which bidders and contracted providers may appeal or dispute a decision made by the community service provider. 6. Applicant/Client appeals shall be addressed as specified in Section 7 of the Division of Aging Home and Community Care Block Grant Manual for Community Service Providers, dated February 17, 1997. 7. Community service providers are responsible for providing or arranging for the provision of required local match, as specified on the Provider Services Summary, (DOA-732). Local match shall be expended simultaneously with Block Grant funding. 8. Providers expending $300,000 or more in federal financial assistance through the Home and Community Care Block Grant, or in combination with other federal funding shall receive an annual independent audit which meets the requirements of the Division of Aging Program Audit Guide for Aging Services, applicable North Carolina General Statutes and Local Government Commission requirements, and OMB Circular A-133. For-profit community service providers shall have an annual compliance audit which meets the. requirements of A~133. The audit shall be performed within nine (9) months of the close of the provider's fiscal yeaz. Upon completion of~the audit, non- profit and for-profit providers shall provide a copy of the audit report and any opinion letter simultaneously to the County and the Area Agency. Federal funds will not reimburse the cost of a single audit if the total of all federal funds expended by the provider is less than $300,000 9. Compliance with Equal Employment Opportunity and Americans With Disabilities Act requirements, as specified in paragraph fourteen (14) of the Agreement for the Provision of CouniyAgin~-Based Services (DOA-735) shall be maintained. 25 DOA-734 (revised 1/99 10. Providers In-Home Aide, Home Health, Housing and Home Improvement, and Adult Day Care or Adult Day Health Care shall sign and return the attached assurance to the area agency on aging indicating that recipients of these services have been informed of their client rights, as required in Section 314 of the 1992 Amendments to the Older Americans Act. 26 Standard Assurance to Comply with Older Americans Act Requirements Regarding Client Rights for Agencies Providing In-Home Services through the Home and Community Care Block Grant for Older Adults As a provider of one or more of the services listed below, our agency agrees to notify all Home and Community Care Block Grant clients receiving any of the below listed services provided by this agency of the their rights as a service recipient. Services included in this assurance include: -In-Home Aide -Home Care (home health) . -Housing and Home Improvement -Adult Day Care or Adult Day Health Care Notification wilt include, at a minimum, an oral review of the information outlined below as well as providing each service recipient with a copy of the information in written form. In addition, providers of in-home services will establish a procedure to document that client rights information has been discussed with in-home services clients (e.g. copy of signed Client Bill of Rights statement). Client Rights information to be communicated to service recipients will include, at a minimum, the right to: -be fully informed, in advance, about each in-home service to be .provided and any change in service(s) that may affect the well-being of the participate; -participate in planning and changing any in-home service provided unless the client is adjudicated incompetent; -voice a grievance with respect to service that is or fails to be provided, without discrimination or reprisal as a result of voicing a grievance; . -confidentiality of records relating to the individual; -have property treated with respect; and -be fully informed both orally and in 'writing, in advance of receiving an in-home service, of the individual's rights and obligations. Client Rights will be distributed to, and discussed with, each 'new client receiving one or more of the above listed services prior to the on-set of service.
