HomeMy WebLinkAbout2004 S Health - Active Seniors Project Agreement with TJCOG` ~~ la ~~/aooy AN AGREEMENT BETWEEN ORANGE COUNTY HEALTH DEPARTMENT AS A PART OF THE COUNTY OF ORANGE AND TRIANGE J COUNCIL OF GOVERNMENTS THIS AGREEMENT, effective November 1, 2004 through June 30, 2005 by and between the Orange County Health Department, hereinafter referred to as OCHD and. Triangle J Council of Governments, hereinafter referred to as TJCOG. The Orange County Health Department is part of the County of Orange. WITNESSETH: WHEREAS, TJCOG was an active partner in the development of the Active Seniors Project as submitted to the North Carolina Department of Health and Human Services by the OCHD, and WHEREAS, TJCOG and the OCHD agreed that the OCHD would act as the fiscal agent for the Active Seniors Project in accordance with the requirements of the North Carolina Department of Health and Human Services, Division of Public Health, and WHEREAS, OCHD has been awarded one-time funding to carry out the elements of the Active Seniors Project, and WHEREAS, TJCOG desires to provide services as outlined in the Active Seniors Project Proposal, under the terms and conditions set forth in this Agreement and consistent with the intent authorized by the funds provided by the North Carolina Division of Public Health, NOW, THEREFORE, in consideration of the premises and of the following mutual promises, covenants, and conditions, the parties agree as follows: 1. TJCOG will perform the following activities in an effort to increase the opportunities for physical activity for residents using or living in the areas around Homestead Park, the Southern. Human Services Center and the anticipated Southern Orange Senior Center: • Conduct a review of all relevant local plans and prepare a summary document of findings by January 2005. • Prepare a GIS-based map of current and planned facilities and potential physical activity sites in a radius to be determined around the planned Southern Senior Center by January 2005. • Participate as a project leader in the selection of an assessment tool and the development of a community training to conduct the assessments in the neighborhood surrounding the Southern Human Service Center by March 2005. ~~ • Compile and analyze the results of the community assessments, develop a report, and distribute to Project Team members by April 2005. • Participate with the Project Team in formulating recommendations and preparing the final report, including graphics and layout of the final report by April, 2005. • Create draft of trail map and produce a final trail map for publication by May 2005. • Participate in the creation of the Active Seniors Project Toolkit by June, 2005. 2. TJCOG will expend the grant funds as agreed on the following items: • $15,860 for technical support of the Land Use Planning Manager • $100 for GIS analysis map plots • $250 for GIS trail map plots, framed trail reference maps and handout maps • $180 for travel 480 miles X $0.375/mile • $50 for research documents • $100 for the production of CDs for the project tool kits 3. TJCOG will expend funds by Apri130, 2005. 4. TJCOG will submit a project report by December 31, 2004 and March 31, 2005 that includes progress on activities to date and total expenditures to date by line item. 5. TJCOG will submit an end of project report by June 30, 2005. The final report will include a program activity report and a final expenditure statement. Copies of receipts for materials purchased will be included in the final report. 6. TJCOG will submit an invoice for the full agreement amount no later than Apri130, 2005. 7. OCHD agrees to provide financial support to the TJCOG in an amount not to exceed $16,540 for the Active Seniors Project, payable within thirty (30) days of OCHD's receipt of an invoice. 8. This Agreement shall become effective on November 1, 2004 and continue in effect through June 30, 2005. 9. This Agreement contains the entire understanding of the parties and shall not be altered, amended or modified, except by an agreement in writing, executed by the duly authorized officials of both parties. 10. The laws of the State of North Carolina shall govern the validity and interpretation of the provisions, terms, and conditions of this Agreement.
