BACKGROUND: At the joint meeting between the Board of Health and the Board of County Commissioners on March 4, 2010, there was significant discussion regarding the dual service model for dental health services, spurred by the impending expiration of the Carr Mill lease. That lease is set to expire June 30, 2011 and not later in the year as had been previously thought. The Board of Commissioners requested that the Department consider various options for service provision to County residents along with a financial analysis. The Board of Health studied the issues associated with providing dental clinical services to low income and uninsured Orange County residents who do not otherwise have access to dental clinical services. Attachment 1 is a memorandum sent to the Board of Commissioners on April 27 with the Board of Health's recommendations regarding the provision of dental clinical services. Also included are the attachments to that memorandum. It is critical for a decision to be made so that adequate time is available to inform patients and the community where services will be delivered after July 1, 2011. If Recommendation #1 below is accepted by the BOCC it will also be necessary to allow enough time to reconfigure the space at Whitted to accommodate the transfer of equipment from Carr Mill. If Recommendation #3 below is accepted, it will provide early budget preparation direction for staff. Recommendation #4 below does not preclude the development of a new single centralized clinic should circumstances with an academic partner such as the community college or UNC present itself. Recommendation #4 is for future financial planning purposes. a FINANCIAL IMPACT: The financial impacts of each option are presented in Attachment 3, Option Revenue and Expenditure Spreadsheet. RECOMMENDATION(S): The Manager recommends that the Board: 1) Exercise Option 2 as recommended by the Board of Health with a start date no later than July 1, 2011 for consolidation of services. Option 2 is a single 8 chair clinic located at the Whiffed Human Services Center utilizing the current staff complement, transitioning those being served at Carr Mill to the Whiffed site by providing transportation vouchers; 2) Direct staff to communicate with Carr Mill management of the County's intent not to renew the Carr Mill lease; 3) Exercise Option 3 as recommended by the Board of Health with a start date of July 1, 2012; authorizing the inclusion of three additional staff in the health department budget for FY 2012-13; and 4) Include capital costs for the construction of two full time dental clinics, one in northern Orange and one in southern Orange in the County Capital Improvement Plan so that the clinics may begin services no later than July 1, 2021. Services Report and Recommendations Date: April 27, 2010 This memorandum and report are in follow-up to the joint Board of Commissioners and Baard of Health meeting held in early March. During February, March and April the Board of Heaith studied the issues associated with providing dental clinical services fo !ow income and uninsured Orange County residents who do not otherwise have access to dental clinical services. Attached to this memorandum are some of the documents that were used by the Soard of Heaith in studying this issue. The following factors were considered in making recommendations: • Provides dental clin(gl services to bath Medicaid and uninsured Orange County residents . • Preserves access to comprehensive dental services far Medicaid and uninsured residents - • Enables the most efficient service delivery • Considers the distribution of current patients and travel distances • Considers the current county's financial status for providing services The Board of Health's primary recommendation for optimal access to dental services for the underserved population is to operate two fully staffed dental clinics. Because of the current financial status of the county, the Board formulated a series of staged recommendations/decisions that it wishes the Board of Commissioners to consider far action. Recommendation 1: Exercise Option 2 beginning July 1, 2011. (See Attachment 1- Optionsfor Providing Dental Treatment Services in Orange County) Option 2 is a single 8 chair clink at the Whined Building utilizing the current staff complement, traiisitioning those being served at Carr Mill to the Whitted site by providing transportation vouchers. - This allows sufficient time for renovation of Whined for denta! services after the personal hearth clinical space is renovated and allows an orderly transition from providing services at Carr Miil prior to the (ease expiration {October 2011). Recommendation 2: Facercise Option 3 beginning July 1, 2012. (See Attachment 1) Option 3 is a single 8 chair clinic at the Whiffed Building with increased staff {additional dentist, hygienist, and dental assistant). This maxim'szes the services that can be delivered for an 8 chair clinic. Exercising this option allows for full transition to a single location and provides time to recruit new staff. Because of recruitment and orientation times, new staff may not be fully productive during the first year of operation. Recommendation 3: Exercise Option 1 no later than the beginning of July 1, 2021. Option 1 is two fait time clinics at different locations (one south and one north) in the county. This option provides the best access to comprehensive dental services for county residents and is the preferred option of the Baard of Health. 4 Memorandum to BOCC April 27, 2010 Page 2 Delaying Option 1 for ten years, white not ideal, allows a true test of the one site concept for accessibility to the population, allows the county's finances to fully recover, and allows for a deliberate dialogue and engagement for site selection{s) in light of an additional 10 years of population growth.
