VEHICLE LEASE AGREEMENT

hereinaftm referred to as "Orange Cour~t}+' and Wake Opportwities, Inc., hereinafter
referred to as "Heed Stare".
The purpose of this Ageemmt is to outline the condhioru under which Orange Couvty
and Heed Start will cooperatively provide human services transportation through Ne
Ornge Pubhc Travsportauo¢ service.
OF ORANGE COUNTY
1. To lease the following idmtified or similar sissd vebiWe(s) az the hows spx5~:

2. io provide altimvrmce, fuel, maivtemnce, repairs and replamment for each leased
veNcle.
3. To provide back-up vehicles for the two Head Stan buses used to provide the service.
SPECDTC RESPONS1R111TIE5 OF HEAD START
1. To reimburse Orange COUntyazerate of840 per van nine,545 per mirdbus mile and
$ 55 per bus mile witbin 30 days after billvig.
2. To operate and schedWe the use ofthe leased velvcle(s) on behalfofhumav services
ctimts and approved groups N accordance with the Ormge County Hrrmm Services
Transportation Board's policies and Orocadures.
3. io park all leased vehicles N the Orange Public Tmvsponation parking lot az 300
West Tryon Svcet, HJlsborough when not in use, unless otherwise authorized.
6. To wmplete all required vehicle operation reports and submit weekly to the
Trareportatiov Supervisor et 300 Wen Tryon Sveet.
5. To comply with all applicable federal regWadm 6overrdng workplace and-drug and
alwhol programs m the transit wdustry. The FNemI Transit Administration ofthe
U. S. Department of Tmmponation has published 49 CFR Part 653 and Part 654, as
amended, that mandate urine drug testing and breath alcohol testing for safety
e~itive positions and prohibits performavca of safery seosi[ive funcrions when these

DRIVER AGREEMENT

Orange on behalf of the Department on Aging's Orange Pubhc Trmuportetion service,
hereinafter referral to az "Orange County" end Wake Oppornutitiee, Ivc., herehafter
referred w az "Head Start ".

4. To pmvidethe Transportation Supervisor wrineo vot5cmiov ofany changes to the
fixed route at least tluee working days prior to the effective date.
