ORANGE PUBLIC TRANSPORTATION
VEHICLE LEASE AGREEMENT
This Agreement is enteral into this first day of July, 1998 by aad behveen the Coutty of
Orwge on behalf of the Department on Aging's Osage Public Tramponation servi¢,
hereinafiu referted to as "Orwge County" wd OrangcPerwn-Chazham Mental Health,
hereiaafler re[ ed to as "OPC".
The purpose of W s Agreement is to outline the conditions under which Orange Coutty
and OPC will wopaatively provide Human services transportation through [he Orange
Pubhc Transportation service.

1. To lease the following identified or similar sued veWcle(s) ache hours specified:
CAT 6 ffi 25 - 14 Passenger Van
CAT 23- 18 Passengu Minibus with lift
2. To provide all inswwce, fuel, maintenance, repairs and replacement for each leased
veldcle.
SPECIFIC RESPON$®B.TTIES OF OPC
L To reimburse Orange County et a rate of 8.40 per van mile, t.45 pu minibus and bus
mile within 30 days after billing.

3. To park all leased vehicles in Ne Orange Public Transportation parking lot at 300

4. Tn complete all required vehicle operazion reports and submit weekly to the
Transportation Supervisor at 300 Wee[ Tryon Street.
5. Toestablish criteria fbr alieoo Cider eligibaity as follows. Q)aveilablliry oP seats; (2)

U.S. Department of Trwsponation has published 49 CFR Part 653 and Pan 654, as

Both parries agrce that the emergenry prowdures will be az follows: Two-way radios wJl
be azsigned in each vehicle with the base monitored by the Department on Aging In the
vent of a mechaNCN emergency, the driver will radio to base for help- Incase ofa
medicN emergency, the driver wJl frst radio m 911 for aid and then to base. The base
operator will be responsible for contacting OPC. At m time wN the driver leave ridrrs
unatteMed.

DRIVER AGREEMENT
This Agrcement is entered into this &n day of Iuly, 1998 by end between the Comry of
Orange ov behalf of the Department on Agmg's Orange Public Travsponatiov service,
hereinafter referred to az "Orange Couvty" end Omvge-Person-Chatham Mental Health
Center, hereinafter referred to az "OPC ".
The propose of this Agreement is m outline fie wnditions under wNch Orange Couvty
and OPC wJl cooperatively provide human services transportation through the Orange
Pubbc Tmvsportadon service.
SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY
1. To Drovide ou certified driver for warier service among OPC sites and daJy pick-up
and tako-home of AdWt Day Treatment clients. This driver will be supervised by
- Orange County.
2. To provide back-up diners N case the assigned driver is not available.
To employ a Tmreportatiov Supervisor to handle any problems and to assure that
~ diners are available.
4. To comply with OPC vansportation policies and procedures as well az state standards.
5. To comply with all applicable federal regulations govurwng workplace anti-drug and
Wwhol programs N the transit indunry. The Federal Transit Admiisvaziov (FTA) of
