PURPOSE: To consider making new appointments to the Board of Health.
BACKGROUND: The Clerk's office has received recommendations from the Board of Health
for Dr. Paul Chelminski, Dr. Michael Carstens, Tony Whitaker and Christopher Cooke. If
appointed, Dr. Carstens, Mr. Cooke and Tony Whitaker will be serving a full term ending
6/30/2012. If appointed, Dr. Paul Chelminski will be serving a partial term ending 6/30/2010.

Re: Recommendation for At-Large Position
At the June 24, 2009 meeting of the Orange County Board of Health, the
Board unanimously recommended Christopher Cooke for consideration
to the At-Large position on the Board of Health recently vacated by
Sharon Freeland who had declined a second term.
Mr. Cooke's skills as a communicator and his 20-year work experience in
public health would bring a valued added dimension to the Board.

Quality Improvement Initiative: Using Internet-based patient registries all internal

medical management within UNC Internal Medicine Clinic. Residents were instrumental
in developing management algorithms. They are engaged in Plan-Do-Study-Act cycles
that are patient outcome-based. Initiative satisfies internal medicine core competencies
of practice-based learning and system-based practice. 2006 to present. Same Day
Clinic, University of North Carolina at Chapel Hill Internal Medicine Clinic: In 2005-6, was
charged by the leadership of the Department of Medicineand Division of General Internal
Medicine to structure and develop the Internal Medicine Same Day clinic as a model
venue for delivering efficient, high quality acute care within a teaching curriculum. 2005
to present. Contiuity of Elective for Internal Medicine Residents: Developed and
implemented continuous two month outpatient rotation for Internal Medicine Residents.

Over 20 years with the UNC School of Public Health and the NC Division of Public
Health. For 10 years I did continuing education and training for state and local public
health agencies. In 1995, I designed and became the director of the NC Public Health
Awareness Program. Before working in public health, I did clinical counseling and
training with the Wright School Residential Treatment Program for Adollescents in
Durham. In the mid-1970s, I worked for two years in the Orange County School System
in a dropout prevention program. I currently teach social marketing at the Leadership
Program at the UNC School of Public Health and do private consulting in marketing and
communication with public health agencies and programs agencies across the state.

founded and directed the North Carolina Public Health Awareness Program for almost 10
